**Hours**: 37.5 per week

**Hourly Wage**: $39.00 per hour plus RRSP and benefits after probationary period has passed.

The Health Navigator will ensure a high standard of person-centered service delivery. The Health Navigator will use trauma-informed practice to overcome barriers in all of CWAVS’ housing programs to accessing care; helping to fill the gap for underrepresented populations by coordinating with paraprofessionals to ensure their presence, funding and contracts allow them to support our client’s needs at the program site.

The Community Health Services Manager will also support a host of community-based health programs aimed at ending toxic drug-related fatalities through direct support and strengthening substance users’ social determinants of health. These programs include Peers-led Outreach, education and community led initiatives.

Additionally, this position will provide guidance in programs with clients designated as “complex care”. The Complex Care Housing supports represents a suite of services and supports aimed at ensuring people with significant mental health, substance use, and other health complexities attain safe and stable housing or shelter as a foundation to thrive.

The Manager plans, organizes, controls, integrates, and evaluates the work, and oversees and manages these areas and their related assets, including:

- Community health programs and clinics/initiatives
- Mental health and wellness programs, and harm reduction programs
- Community assistance and non-insured health programs (complementary health services such acupuncture, etc.)
- Family support and outreach programs.

Primary goals are for supporting the development of community health and interventions that enhance the quality-of-life and quality of place for community members. The work also includes ensuring the development of and the on-going mentorship of a community of practice for assigned work units, through the lens of inclusivity, decolonization, and Indigenization.


- The Health Navigator will oversee the Client Navigator at Cedar Branches and expand the role into all housing sites.
- Be a go-to for the Client Navigator on complex client files.
- Create strong relationships with other agencies on the Island to have a solid referral resource list.
- Work with the Paraprofessionals to ensure they have what they need in the spaces and be their go-to for any questions, concerns, scheduling, or programming issues/ideas.
- They will coordinate and liaise with relevant community partners to create meaningful partnerships that will support the physical, mental, emotional, and spiritual wellness of clients through paraprofessionals and relevant providers.
- Attend health related community groups, workshops, committees, and other relevant activities as directed by the PHRD to foster collaboration and bring intersectional perspectives and solutions to CWAVS’ and the community.
- Work with the housing managers to ensure they have what they need in terms of health care to best meet their client’s needs.
- Work with cultural safety at the forefront. Understanding of cultural humility, Indigenous practices surrounding health and trauma, and a willingness to learn about all cultures represented in the Valley to respect their cultural values and beliefs.
- Oversight of trauma-informed intakes, documentations, client notes, statistics, and reporting
- Working with Housing Managers on any relevant training and collaborating on next steps for integrating health and housing into CWAVS’ with the PHRD at the Housing Meetings
- Integrating social workers, mental health workers, pharmacies, and other providers into housing
- Develop, coordinate and implement care plans/discharge plans with hospitals and work closely with Aboriginal Liaison Nurse, social work and hospital-based teams to ensure optimal health outcomes as possible
- Research, identify and analyze current social determinants of health of community members and work proactively with relevant CWAV programs and outside agencies to eliminate the gaps and barriers to care.



**Education, Training and Experience**: 

Master’s or equivalent in health related or social service sector

Current First Aid Certificate 

Non-Violent Crisis Intervention Training
