Hospice Niagara, a not-for-profit charitable organization that focuses on improving the quality of life for people living with life-limiting illnesses, death, dying, grief and loss, is seeking a Clinical Navigator. This is a part-time, hybrid position which will require you to work on-site at Hospice Niagara in St. Catharine's.

**Position Summary**:
As a member of an interdisciplinary Palliative Shared Care Team, the Clinical Navigator & Client Service Coordinator (“Clinical Navigator”) coordinates referrals to Hospice Niagara team members, and Nurse Practitioners and Physicians working members of this team. This position also provides triage for the Shared Care Palliative Care Outreach Team. This position also provides individual assessments for families in underserved areas, with the goal of linking the family to other Hospice Niagara Services. This position provides support and back-up for all other administrative positions, including physician phone calls, billing and file maintenance.

**Key Responsibilities**:

- Prioritizing all incoming calls from patients, families, community agencies and physicians, ensuring team members are up-to-date on client and family care at all times
- Determining and interpreting the needs of clients and families, triaging needs and informing the physician’s and care providers of the order of priority
- Follow-up to ensure services and information has been received by all parties and services are in place
- Managing client files, ensuring information is up-to-date at all times
- Preparing clinical rounds lists, updating client information
- Leading clinical rounds, ensuring confidentiality of client information
- Preparing accurate statistical reports as required
- Transcription of physician visit information to medical record
- Notifying team members and updating family file when situations change including the death of a client to ensure bereavement services are in place
- Assessment of the needs of client and family needs in assigned underserved jurisdictions
- Referring clients and family members in underserved jurisdictions to Hospice Niagara volunteer and other services
- Providing administrative support and/or back-up for other team members, whether or not their employment is with Hospice Niagara
- Other duties as assigned

- Demonstrated skills in medical terminology and transcription
- Excellent interpersonal and conflict resolution skills
- Knowledgeable of support services and resources in the community
- Comfort in working with a number of software program for the purpose of statistical reporting
- Excellent written and verbal communication skills
- Computer literacy level to manage Team operations: Word, Excel, Access
- Exhibit a professional, compassionate and caring approach
- Demonstrate strong active listening and interpersonal communication skills
- Ability to develop relationships based on trust, compassion and empathy
- Effectively set appropriate boundaries and understand the limitations of volunteers in client care
- Demonstrate a high degree of self-direction and self-motivation
- Demonstrate a healthy adjustment to loss and exercise ongoing self-care

**Academic, Training, Knowledge & Experience**:

- 3-5 years’ experience working in hospice palliative care
- Registered Practical Nurse or other health related diploma preferred
- Experience working and collaborating within a multidisciplinary team
- Experience working with local organizations and primary care
- Vulnerable Sector Police Check (favourable, within past 12 months)
- First Aid/CPR (preferred, must be taken within 12 months of hire)
- Safe Food Handling (preferred, must be completed within 12 months of hire)
