The Elgin Ontario Health Team (Elgin OHT) is made up of organizations, clinicians, and community members working together to improve the health and wellbeing of the people of Elgin. Together, we are reimagining how to provide care in a more integrated, coordinated way that better serves and supports our community. We’re looking for a systems-thinker who has a passion for population health, health equity, and quality improvement, with the ability to motivate others and work directly with service providers and community members to impact change. For this role, we are also seeking someone with working knowledge of the Elgin community and primary care.
**Position Summary**
Reporting to the Elgin OHT Executive Director, the Population Health and Quality Improvement Lead will advance the use of population health and quality improvement approaches within the Elgin OHT. The incumbent will provide both strategic and operational support, with a particular focus on engagement with primary care providers.
**Key Responsibilities**
- Lead the evolution and implementation of the Elgin OHT Evaluation Framework and Performance Measurement Plan, including the related scorecard.
- Lead the annual development of the Elgin OHT Collaborative Quality Improvement Plan and related integration into applicable areas of work/initiatives.
- Lead a local table of data experts to manage, collect, and analyse clinical and non-clinical data at the community and population health level to a) understand the changing needs of Elgin’s attributed population and, b) find opportunities for quality improvement, program delivery, and achievement of strategic priorities by better understanding subpopulations of focus.
- Coordinate everyday tasks and activities related to quality improvement initiatives across the Elgin OHT, including working directly with clinicians on change management and implementation strategies.
- Work with the OHT clinical leads to engage primary care providers in improvement efforts and other OHT activities.
- Using a population health lens, support the development of initiatives and co-design within the OHT in partnership with other team members.
- Other duties as required.
**Qualifications and Experience**
- University degree or equivalent training/experience in epidemiology, biostatistics, health administration, health informatics, health sciences, public health, quality improvement, practice facilitation, or other related field
- Three or more years of experience in a healthcare environment in a quality improvement and/or change management capacity
- Experience in applied health system measurement and evaluation
- Awareness of data sources for healthcare measures and ability to engage partners to define and collect data across multiple data sources.
- Working knowledge of the Elgin community and primary care, with experience engaging primary care providers preferred.
- Experience translating knowledge for various audiences
- Working knowledge of change management principles and practices
- Strong project management skills, including prioritizing and managing multiple projects with complex demands and short timelines
- Proficient computer skills for report and presentation preparation, data analytics

Position involves regular travel within Elgin County. A valid driver’s license and access to a reliable vehicle are required.
