**Department**: Home Support

**Qualifications**:

- Bachelors or Master's degree in Social Work, Occupational Therapy, Public Health, Gerontology or other related discipline
- Registered with relevant professional college/association
- Experience in mental health, geriatrics, healthcare, community care
- Experience working in Long Term Care setting is preferred.
- Project participation and/or management
- Must have own Behicle or access to reliable transportation

**Major Responsibilities**:

- Responsible for providing assistance to units with daily routines to residents/clients with activities of daily living
- Completes Individualized Holistic Assessments
- Develops Personalized Care Plans
- Coordinates/Participates in Educational Outreach
- Collaborates with Stakeholders
- Leads/Collaborates on Advocacy Efforts
- Project tasks involving project planning, reporting and evaluation

**Major Tasks**:

- Works collaboratively with others to conduct holistic client assesments that include social determinants of health
- Uses aggregate data and works collaboratively with others to identify, organize and facilitate social activities, support groups and educational workshops that foster wellbeing, a sense of community and help overcome social isolation among seniors
- Identifies unmet needs and collaborates with system partners to develop and or deliver educational materials on health promotion, health literacy, disease prevention, harm reduction, and navigating systems to empower individuals with knowledge and skills
- Links individuals to the appropriate resources (ie: Care Coordinator, Community Relations Worker) to facilitate referrals and access to community programs, social services, and health care options.
- Develops individualized care plans with clients and other care providers
- Advocates to address specific challenges and engages in system-level advocacy efforts to address issues affecting the collective wellbeing of clients
- Provides project support related to data collection, reporting, planning, monitoring and evaluation
- Communicates effectively with clients and stakeholders, and maintains accuate and timley documentation of assessments, care plans and interventions
- Regularly monitors progress, adjusting care plans as needed
- Promotes selfcare and wellbeing, providing guidance on healthy lifestyle choices and encouraging engagement in activites that contribute or physical and mental health
- maintains appropriate documentation and case notes as needed
- Completes other duties as required

**Salary**: As per the Northwood Scale

**Hours**: 40 Hours Biweekly ( Days/Evenings )
