**Indigenous Health and Wellbeing Coordinator (IHWC)**

**Job overview**:
Reporting to the Director, Clinical and Allied Health Services, the Indigenous Health and Wellbeing Coordinator (IHWC) will provide health promotion and navigation services to clients of Chigamik CHC and support Indigenous initiatives across the organization, inclusive of the Traditional Healing Program.

The primary tasks and objectives of the role are as follows:

- oversight of Indigenous cultural services, programs, and initiatives
- fostering and developing relationships with community partners
- assisting clients to connect with programs and services
- provide direct client support and referrals as needed
- implement Indigenous cultural health promotion practices across the organization

As a member of a multidisciplinary team, the IHWC will work in collaboration with both the Allied health and Clinical teams to develop initiatives that foster inclusion and facilitate the delivery of programs for clients who are marginalized, at risk of a chronic disease, and/or in need of mental health/social support, with a focus on supporting FNMI clients and community members.

Desired Qualifications and Requirements:

- Post-secondary education in health promotion, system navigation, social services, social work, addictions, mental health, community development or a related field.
- 3+ years relevant experience in indigenous health promotion, counselling, or navigation.
- Knowledge of health promotion strategies and community resources.
- Knowledge of and experience working with Indigenous services and agencies.
- Knowledge of or familiarity with regional Indigenous cultural practices and beliefs and with traditional Indigenous healing practices.
- Knowledge and understanding of reconciliation initiatives at the local community, provincial and federal levels.
- Experience working effectively with culturally, economically, and socially diverse clients and/or community organizations.
- Experience working with a broad range of community members, including youth and elders.
- Experience with Indigenous people and a deep understanding of the unique challenges facing Indigenous people and communities.

**Aperçu du poste**:
Relevant de la Directeur, Services cliniques et paramédicaux, la coordonnatrice/le coordonnateur de la santé et du bien-être autochtone (CSBA**) **offre des services de navigation et de promotion de la santé aux clients du CSC Chigamik est appuie les initiatives autochtones dans l’ensemble de l’organisme, y compris le programme de guérison traditionnelle.

Les principales tâches et objectifs du rôle sont les suivants:

- Superviser les services, programmes et projets culturels autochtones;
- cultiver et entretenir des relations avec les partenaires communautaires;
- aider les clients à entrer en contact avec les programmes et services;
- fournir le soutien direct aux clients et les renvois au besoin;
- mettre en œuvre les pratiques de promotion de la santé culturelle autochtone dans l’ensemble de l’organisme.

En tant que membre d’une équipe multidisciplinaire, la/le CSBA travaille en collaboration avec l’équipe clinique et l’équipe paramédicale en vue d’élaborer des projets qui favorisent l’inclusion et facilitent la prestation de programmes pour les clientes et clients marginalisés ou à risque de maladie chronique ou ayant besoin de soutien social ou de santé mentale, en mettant l’accent sur la clientèle et les membres de la communauté PNMI.
