**HOURS: DAYS, EVENINGS, AND WEEKENDS
new and innovative Mobile Acute Care of the Elderly (MACE) Program through collaboration with other members of the
Allied Health team. The Geriatric Assessor (GA) will support the work of the MACE team, as a core member. Duties will
include completion of Interdisciplinary Geriatric Assessments (IGA) / Comprehensive Geriatric Assessments (CGA’s),
development of individualized care planning in Expanse, and ensure ongoing re-evaluation of the clients we serve,
updating the plan of care to meet their individualized needs. The GA’s target population is older adults who are at risk of
becoming ALC, are designed ALC, or whose complex discharge needs put them at risk of re-admission to hospital, again
with an increased at risk of ALC designation.

**QUALIFICATIONS**

**Education **

Minimum four (4) year Bachelor’s degree in a health related field, from an accredited university. 
Current Certificate of Registration in good standing as a Health Professional with an Ontario Professional Association

or regulatory college 

Completion of the Provincial Geriatrics Leadership Ontario (PGLO) Common on-boarding (Levels 1 & 2), or
willingness to complete within the year 

Completion of Geriatric Certificate Program (McMaster university) required, or willingness to obtain within 2 years 
Current Gentile Persuasive Approach (GPA) Certification required

**Experience/Knowledge/Skills **

Relevant experience working within a health care setting with a focus on older adults. Preference is to those with
experience within the past 5 years. 

Experience or demonstrated understanding of the Comprehensive Geriatric Assessment framework, and geriatric
goals of care. 

Demonstrated excellence in analysing assessment findings and implementing individualized treatment plans
based on the Comprehensive Geriatric Assessment framework. 

Demonstrated knowledge of Specialized Geriatric Services across the continuum - including inpatient services,
community clinics, programs, and resources. 

frameworks, supporting the optimal assessment and treatment intervention. 

Demonstrated skills in management of geriatric syndromes (i.e. polypharmacy, falls, functional decline, acute/sub
- acute cognitive changes, bowel/bladder, and nutrition/hydration, etc.). 

Demonstrated excellence in a leadership role required. 
Must demonstrate excellent level of interpersonal skills, written, and verbal communication skills, with

consideration of the intended audience. 

Demonstrated person and caregiver focused care, and supported by best evidence-based clinical practices within
respective scope of practice. 

Ability to work both independently and collaboratively with all members of the multidisciplinary team, clients,
families, regional support services and community partners required. 

Ability to provide health teaching to individuals and groups required 
Must be committed to continuing education on an annual basis and demonstrate commitment to quality

improvement required. 

Must demonstrate basic computer knowledge. 
Must demonstrate commitment to ensure personal safety, safety of fellow workers, patients, volunteers, and

visitors. 

Ability to support and contribute to a culture of safety and prevention of adverse health events required. 
Must demonstrate good attendance and work record. 

**_A formal French test will be conducted to evaluate the proficiency level._**
Criminal Reference Check including the Vulnerable Sector Check recent within six (6) months is a requirement
