**Safe House Follow up Coordinator, Seniors’ Safe House**

The **Follow-up Coordinator** position is **part of the Seniors’ Safe House team and provides follow up care to clients who have recently been discharged from the Safe House program**. The Coordinator will assist with the discharge process and **continue to assess and advocate for the needs of the clients being discharged and facilitate their transition into the community.**

**QUALIFICATIONS, SKILLS and EXPERIENCE**
- Must be eligible for registration with the ACSW
- Social Work degree or diploma
- Must provide an enhance criminal record check from the Edmonton Police Service
- Class 5 Driver’s License; Vehicle in good repair with $2 million liability insurance is required
- Experience working with older adults
- Familiarity with social and psychological issues related to elder abuse and neglect
- Knowledge of community resources and services related to older adults and care providers
- Excellence interview, assessment and communication skills
- Knowledge of the legal system
- Self-motived and able to work both independently and as part of a team

**Job Types**: Part-time, Permanent

**Salary**: $23.32-$26.07 per hour

Expected hours: 20 per week
