In 2023, as the regional anchor organization for geriatric care, teaching and research, St. Joseph's Health Care London (St. Joseph's) conducted a review of the provincial landscape of geriatric structures. This review identified opportunities to strengthen our approach to regional supports for older adult health care in Southwestern Ontario through one aligned entity - the Regional Geriatric Program of Southwestern Ontario (RGPSWO). The priorities of the RGP include - clinical excellence, education and capacity building, research and evaluation, capacity planning and advocacy. The RGPSWO has a regional governance structure and this role would require close connection to the executive and operational steering committees.
- This clinical coordinator role will lead regional operations, and continuous quality improvement to improve care for older adults across the geographic region of Southwestern Ontario. The role will focus on managing operations of the following regional programs: Behavioural Supports Ontario (BSO), Geriatric Emergency Management (GEM) Oversite, Nurse Led Outreach Teams (NLOT), Geriatric Ambulatory Access Team (GAAT), and other regional programs where direct or indirect leadership is required to deliver on the priorities of the RGP.
- Further, this role will lead Senior Friendly Care (sfCare) Initiatives within St. Joseph's as well as further develop and manage operations of regional sfCare programming. The Coordinator will have an important operational role in regional and provincial contexts, being a strong advocate for aspects of front-line clinical care delivery.
- Lastly, as education is a priority of the RGP, this role will lead the operational aspects of the RGPSWO education strategy, and provide leadership to the resources employed to deliver education services to the region.
- People Management:
- Lead human resource management activities including recruitment, scheduling, deployment, performance development and performance management (coaching and mentoring), recognition, attendance management, and orientation/training of staff. These activities will be required for the GAAT, NLOT, and RGPSWO Education teams.
- Undertakes workforce planning, sets objectives, ensures daily operations of clinical programs (ex. Appropriate resources, providing direction on how work is completed).
- Ensures professional practice compliance with professional, accreditation, ethical, and legal standards. Assesses models of care within clinical programs and implements changes as appropriate.
- Collaborate with key regional stakeholders to manage indirect leadership accountabilities of regional programs (ex. GEM)
- Coaching and mentoring of direct reports and regional partners as required based on the priorities of the RGPSWO
- Ensure strong partnerships with the academic community of Southwestern Ontario
- Fosters collaborative working relationships with union partners and provides leadership in employee/labour issues, and participates in formal grievance and arbitration processes.
- Supports ongoing regular engagement with BSO leadership and - Enhanced Psychogeriatric Resource Consultants (EPRCs) with clinical and operational issues
- Financial Management:
- Formulate an annual budget and monitor monthly, adjusting as required to maintain a balanced budget. Identify variances and propose mitigation strategies to Director.
- Collaborate with key stakeholders regarding budget requirements for regional service development, including merging/integrating/ transferring funds across the region.
- Participate in advocacy efforts aimed at attaining new and sustainable resources that will enhance care for older adults in our region.

Risk Management and Safety:
- Leads and fosters a healthy and safe work environment and attends to risk management programs aligned with organizational values for staff, patients, partners and visitors.
- Understands all aspects of clinical and non-clinical work-flow and proactively mitigates risk
- Ensures adherence to policy, procedures, and practices including mandatory training and education programs.
- Monitors patient care activity, ensures standards of care, best practice, and legislative requirements are met. Addresses deficiencies in clinical practice of regulated health professionals and, in conjunction with Professional Practice, supports learning plan for direct reports.
- Conducts investigations, documents, implements corrective action for safety/care, deficits/near misses and identifies areas of improvement, seeks expert advice, and develops action plans to prevent or minimize future events.
