**POSITION: Evaluation and Improvement Lead 0.90 FTE**

The role of the Evaluation and Improvement Lead (EIL) includes program and system evaluation and reporting, business planning, change management, and working closely with physician clinics, particularly for quality improvement. The EIL also works closely with the PCN’s member physicians and their clinic (medical home) staff. The position acts as a champion for change to support the goals of primary care evolution and aligns them to what is important and valuable at a clinical level. The EIL will develop and implement evaluation plans for PCN programs and support improvement projects within the medical home. Relationship building and data collection will inform the strategic direction for physician members as well as inform PCN program planning.

**QUALIFICATIONS**:
**Education and Training**:

- Undergraduate degree in healthcare, health promotion, business, and/or the social sciences, or related field. A graduate degree is considered an asset.
- Change management training/education.
- Process improvement training (e.g., LEAN) is an asset.
- Evaluation mindset. Specific training is an asset.
- Comfort with leading and working with multidisciplinary teams. Able to adjust communication style depending on the audience.

**Experience**:

- 2 or more years experience in change management and/or process improvement.
- Experience with group facilitation and comfort in presenting to large groups.
- Experience managing projects and tracking deliverables.
- Experience in evaluating complex programs and projects.
- Experience working with physicians/engaging with physicians is preferred.
- EMR experience and advanced experience in M365 is an asset.

**Skills and Attributes**:
A clear Criminal Records Check, along with a valid driver’s license and access to reliable vehicle is required.
