**Job Description**:
Home and Community Care Support Services North East offers a wide-range of quality health-care services and resources to support people of all ages at home, school or in community. In addition to planning, delivering and coordinating care for thousands of people each day in Northeastern Ontario, we also manage eligibility and admissions to long-term care homes, short stay respite, assisted living, and adult day programs. **Patient Care Contracts Specialist**

**Job Posting #032-24**

**Location: Flexible to be located within any HCCSS North Region with preference to applicants from the North East.**

Reporting to the Manager, Patient Care Contracts, the Patient Care Contracts Specialist is responsible for assisting in enhancing clear accountabilities and effective relationships between Home and Community Support Services North East and Service Providers and achieving high quality outcomes for patients.

**K**EY ACCOUNTABILITIES**:

- Facilitates orientation for new providers involving various internal departments;
- Reviews and analyzes reports and information from multiple sources to ensure adherence to contractual and provincial standards and to identify trends;
- Assists Manager with issuing of quality reports, data assessments and with monitoring service provider performance following terms of the contract and provincial standards;
- Provides support to the Manager and/or Director with contract performance improvement initiatives;
- Establishes and maintains effective working relationships with internal managers, staff and external contracted service providers to facilitate contract compliance;
- Creates and distributes Service Provider communications delivering relevant information from various departments to external providers;
- Assists with maintaining and updating Service Provider Portal sites;
- Contributes to a culture of patient safety, which includes respect, accountability, and open communication in a psychologically safe environment;

**POSITION REQUIREMENTS**:

- Relevant post-secondary education or recognized equivalent work experience;
- Minimum 3-5 years of practical and related experience in quality assurance projects and initiatives;
- Knowledge of quality improvement tools, techniques and measures;
- Experience with various statistical methods, tools and analysis;
- Proficiency with a wide range of databases and data extraction tools;
- Experience in Communications considered an asset;
- Considerable hands on knowledge of MS Office including Outlook, Access, Excel, PowerPoint, and Word;
- Understanding of the healthcare system and direct care/service delivery models used in community agencies;
- Experience working with stakeholders from diverse areas and organizations;
- Ability to monitor and achieve specific outputs of service providers;
- Strong presentation skills to facilitate and provide information to internal staff and external stakeholders as appropriate;
- Strong communication skills;
- Ability to display and share information using various approaches;
- Some knowledge of Home and Community Care Support Services North East business practices and objectives;
- Strong coaching and conflict resolution skills;
- Some knowledge of risk management theory and processes;
- Superior oral and written proficiency in English is essential.
- Oral proficiency in French is an asset.
