The Alzheimer Society of Durham Region (ASDR) is a community support service organization that provides programs and services to people living with dementia and their care partners in Durham Region. We work closely with other service providers to ensure quality care and support for our clients to achieve our mission to improve the quality of life of people living with Alzheimer’s disease or a related dementia and their care partners.
We employ administrative and fund development staff, educators, social workers, social service workers, nurses, occupational therapists, recreation staff, and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

As an Intake Coordinator, under the direction of the Manager, Client Services, you will be responsible for managing all incoming referrals for all programs and services, of the Alzheimer Society Durham Region. This role will also fulfill all expectations outlined in agency Policies and Procedures.
**What you will be doing**
Responsibility
- Completes client intake process including receiving fax and web-based referrals and reviewing relevant referral information
- Answers in-coming agency calls and conducts initial phone calls and/or meetings with individuals (people living with dementia and care partners) and attends to walk-in
- Completes client intake process including administering intake assessment to determine referrals to appropriate internal programs & services and redirects inappropriate referrals appropriately
- Provides follow up letters to referral partners as per agency standards indicating the status of their referral
- Schedules client appointments for all client support staff as required
- Maintains case documentation in line with organization and industry standards
- Enters statistical & client data into electronic client database as per organization standards
- Oversight and assistance with the compilation and distribution of various ASDR information packages for all programs
- Distribution of program materials to community partners as required
- Maintenance and inventory of materials and pamphlets available in the office
- Participates in special projects as assigned
- Active participation in agency team meetings, as well as fundraising activities
- Other duties as assigned from time to time
Other Duties
- Perform other duties consistent with the job classification, as required.
- This role may serve as back up administrative support to the organization including front of office support, phone support, and support of walk-ins.
Relationships
- Reporting to the Manager, Client Services, the Intake Coordinator works as part of a multi-member team and has close working relationships with all ASDR program staff. The Intake Coordinator also maintains working relationships with persons with dementia, care partners, volunteers, and students.
**What you will bring along**
Education
- Social Service Worker Diploma or post-secondary degree/diploma in health, social work, social services, recreation or medical administration
- Specific training in dementia studies is considered an asset
Experience
- Minimum of 2 years experience working in a social service agency
- Minimum of 3 years experience working with persons with dementia and their care partners is preferred
Other Knowledge, Skills, Abilities or Certifications
- Excellent verbal and written communication skills in English
- Comprehensive knowledge and understanding of Alzheimer’s disease and related dementias is considered an asset
- Database experience an asset
- Ability to work cooperatively with volunteers and staff
- Excellent organizational & time management skills with the ability to prioritize and manage multiple tasks
- Flexible with the ability to work evening and weekend shifts when required
- Computer literacy with proficiency in Windows environment; strong working knowledge of Office 365
- Proven ability to set individual priorities
- High standard of ethics and confidentiality
- Shows initiative, flexibility and resourcefulness
- Current Ontario driver’s license and willingness to travel within Durham Region
