Job Summary

**Responsibilities**:

- Liaises with participants and families to plan and prepare participants’ program in accordance with goals.
- Supports participants to encourage best possible educational experience.
- Participate in Occupational Health & Safety Committee as requested.
- Orient, oversee and evaluate program volunteers, grant workers, & practicum students in conjunction with the Director of Community Based Services.
- Follow Positive Behaviour Supports as established Individual Program Plan.
- Maintain Professional Boundaries.
- Assist participant in all areas of lifestyle choices, i.e.: relationships, personal appearance, activity choices and lifestyle.
- Respond to concerns raised by participants, members of their support networks in conjunction with the Director of Community Based Services.
- Maintain a high level of personal care support as outlined in the Support Needs Assessment.
- Carry out health care routines outlined in the Support Needs Assessment.
- Transfers and lifts occur according to the Support Needs Assessment.
- Understand and support participant needs and learning style.
- Dependably adhere to work schedule.
- Ensure a high level of cleanliness and organization in all areas of the worksite and grounds.
- Ensure all equipment used by participants, i.e., wheelchairs, glasses, communication devices, etc. are cleaned and maintained as directed.
- Ensure wheelchair securement is done in safe and appropriate manner on all public transit and vehicles.
- Maintain open lines of communication with all co-workers, support networks, funders and stakeholders.
- Respond and problem solve concerns raised by employees in conjunction with Director of Community Based Services.
- Ensures employees have emergency contact information of participants at all times.
- Processes new participant referrals from referral sources including input into Sharevision and creating network files, etc
- Maintains participant status change or end-of-service administrative process.
- Provide administrative support for the completion of participant documentation including Care Plans, Health Care Plans (HCP), Personal Focus Plans (PFP) / Smart Goals and other related documentation.
- Ensure Participant documentation, including electronic and paper files are maintained.
- Liaises with participants and families to plan and prepare participants’ program in accordance with goals.
- Supports participants to encourage best possible experience
- Ensures employees have emergency contact information of participants at all times
- Maintain, review and record participant progress in ShareVision regarding daily progress.
- Administer and manage inbound/outbound mail, including priority post, packages, courier services, and other correspondence.
- Ensure adequate supplies of office stationery, general office supplies, office furniture, and other equipment.
- Ensure the office facilities are in compliance with CARF standards and safety regulations, such as local fire codes, handicap access, and other laws.
- Other related duties as assigned
- Adhere to all policies and follow established procedures.
- Answer and manage inbound telephone inquiries.

**Job Type**: Part-time

**Salary**: $24.00-$26.00 per hour

Expected hours: 20 per week
