Job Title: Life Coach and Community Connector

Position Full Time, 40 hours/week

Gross Salary Starting 65,000 per year

Health Benefits Available after 3 successful completion of 3 month probation period.

Transportation: A vehicle is required as this position requires frequent travel in the community.

Mileage Vehicle is required, reimbursed monthly.

Hours of Work: 40 hours per week, includes day
- time, evenings and weekends, schedule is developed around the support needs of individuals supported,

Paid Vacation 1 year
- 15 days, 5 years-20 days, 10 years
- 25 days, 15 years 30

The timing of staff vacations at LiveWorkPlay is negotiated to minimize disruption to members.

Desired Background: Individuals must have a college, bachelor or graduate degree in Social Services, Developmental Services, Community Services, Child and Youth Services, Education, Social Work, Psychology, or related field. Other equivalent professional training and experience will be considered.

Other Requirements: Valid First Aid, CPR certification, Crisis Prevention Intervention (CPI)

**Job Summary**

LiveWorkPlay helps the community welcome individuals with intellectual disabilities, autistic persons, and individuals with dual diagnosis to live, work, and play as valued citizens.

Living Supports is responsible to support the individual to live successfully in a home of one’s own. Helping with activities of daily living, such as meal planning, grocery shopping, cooking, laundry, budgeting, participating in mainstream community activities, attending medical and other appointments, as well as other case management duties.

LiveWorkPlay’s approach to supporting individuals is based seven key principles that guide our daily work and they include honouring personal autonomy, person-centred thinking, assets-based solutions, barrier-busting technology, always building bridges, community-first approaches, and authentic valued roles. We refer to this approach as Just Enough Support, which means we are looking for ways to provide each individual with the right amount of support (not too much and not too little) and to maximize opportunities for the individual to do as much as they can themselves, develop reciprocal relationships with family, friends, and neighbours, use and participate in what the community has to offer, and integrate technology to enhance autonomy. The role of paid staff is to support the individual to engage in, access, and sustain the elements of Just Enough Support, and as a last resort, fill in the gaps that the individual, family, friends, neighbours, community services and technology can not fulfill.

Support gaps might include supporting individuals with menu planning, grocery shopping, meal preparation, household management, financial organization, medical appointments and meeting expectations of being a tenant or owner and case management related activities. Living supports also will develop opportunities for the development of natural relationships between neighbours, family members and friends.

Part of supporting someone to be successful in their own home is to be connected and active in his or her community. Depending on the individual’s needs part if this role can include developing opportunities for community participation. Based on the passions, interests and needs of each member the staff member identifies and develops social opportunities in the community. Examples of community participation include volunteer work, recreational activities, classes, courses, community centres, clubs and associations. Once these social opportunities and venues have been staff learn about the culture, rituals, and community expectations of the venue and help members understand them. The staff also spends time identifying and recruiting gatekeepers at each venue. Gatekeepers are valuable people who are influential and powerful who will endorse and acknowledge new people in the venue and positively influence other venue members who will then welcome and include individuals with intellectual disabilities. The staff would then introduce the LiveWorkPlay member to the venue, activity, and gatekeeper and continue to follow-up to make sure success has been achieved.

Living Supports is also responsible for communicating with family members, the LiveWorkPlay staff team, and working in collaboration with community partners and other agencies and with the community at large. Other duties include supporting other organizational projects such as the Golf Tournament, Make a Buzz, professional development, and other projects that may come up throughout the year.
