**Community Facilitator

  • Supported Independent Living Program (SIL)**
    Education: Graduation from a recognized college or university with a diploma or degree in a social services field, such as DSW, CYW, SSW, Psychology, Sociology, or other related studies.

Job Requirements: 1-2 years recent and relevant experience working with individuals with developmental disabilities and dual diagnosis. Current First Aid & CPR and C.P.I certification.

Police Vulnerable Sector Check
- current to within three (3) months. Bilingualism is an asset.

Transportation/Travel:
A valid Driver's License, with a
minimum G2 and must have a reliable vehicle.

Knowledge & Skills:

  • Knowledge of various disabilities and mental health disorders.
  • Solid communication skills both verbal and written.
  • Strong interpersonal skills and demonstrated ability to work independently or in a team.
  • Demonstrated flexibility, creativity, and sound judgement.
  • Computer proficiency in Microsoft Office Suite (Word, Outlook, Excel)

Responsibilities:

  • Provide support to individuals in the community where they live with activities of daily living such as personal safety, healthy relationships, life skills, crisis management and prevention, medical and dental appointments, navigating community resources and housing issues.
  • Advocate on behalf of the individual and/or supporting them to advocate for themselves.
  • Collaboration and/or coordination with families, significant others, community agencies, health care professionals of their choice to find and use services and supports to meet their goals.
  • Complete internal assessments to identify and meet an individual's goals in the areas of functional independence and community participation.
  • Understand and consistently follow where appropriate, any Behavioural, Medical and Crisis Protocols as documented.
  • Support other relevant duties that may be developed or assigned.

Job Type:

Full-time permanent 37.5 hours per week, Salary $27.79, Mon to Fri but maybe required to work evenings and weekends dependent on client or program needs.
