**DUTIES AND RESPONSIBILITIES**:
This role collaborates with the multidisciplinary team and provides client-driven quality, equity-informed health care education and supports in an efficient, equitable manner. The incumbent assesses mental health and wellness needs and assists students and navigate resources and provide education as well as provide case management service to students requiring additional support

The incumbent will work collaboratively with the CHWC team to develop and facilitate various relevant mental health and wellness educational outreach initiatives to support student mental health and wellness programs and activities.

**Direct Student Support**:

- Assesses the mental health needs of students, provides health education, referrals and information about support in community to specialized treatment programs, crisis intervention, hospitalizations as well as re-integration planning post absence from school after a Mental Health leave as appropriate.
- Provides navigation for staff/faculty seeking information about student distress/crisis processes at Durham College as well as guidance in managing student distress.

**Administrative**:

- Continues to implement the goals from the Mental Health Review and CICMH recommendations and utilizing the Mental Health and Well-being Framework to develop and promote health and wellness strategic goals and objectives and contribute to the development of campus mental health and well-being policy and protocols.
- Completes annual reports to help identify the types of incidents, patient satisfaction, needs, the success of interventions and utilize this information to prepare education, training and service delivery recommendations.
- Participates in/on sanctioned committees as Campus Health Wellness Centre Representative.
- Seeks external funding opportunities to support mental health and well-being on campus. Submit proposals and completes reporting requirements for external funding initiatives as appropriate with the CHWC.
- Health Education Outreach:

- Develops and maintains collaborative relationships with community supports and agencies.
- Develops and facilitates health and wellness programs and activities that includes, but is not limited to, delivering workshops, educational videos and seminars, campus-wide outreach events, staff orientation etc.

**QUALIFICATIONS**:

- A minimum of a two-year diploma in Registered Practical Nursing.
- Suicide intervention skills training; CPR-HCP; and valid Mental Health First Aid certificates are required.
- A minimum of five (5) years of work experience in a mental health related field. Experience must include GAD7, PHQ9, Privacy/Confidentiality Act, Substitute Decision Act, Health Care Consent Act, FIPPA, PHIPA.
- Travel required for external partnership maintenance and liaising e ie. to Ontario Shores, Lakeridge Health, Durham Rape Crisis Centre etc.
- Must be able to work both independently and collaboratively as a member within a multidisciplinary health services team.

**Required Skills**:
Starting Rate: $36.19; Four Year Rate: $41.94

Required Experience

Hours: 35 hours per week (1) one hour lunch; Mon - Fri (8:00 am - 4:00 pm; evening shift may be required)
