Your Opportunity:
Reporting to the Placement Manager, North Zone, Seniors Health, the Case Management Coordinator (CMC) is responsible to facilitate and coordinate patient transfers to a Continuing Care Home (CCH). The CMC receives client referrals for access to a CCH and confirms care level based on Home Care assessments, provincial guidelines, knowledge of continuing care services, and clinical judgement. In consultation with the patient/family and the care team, the CMC approves appropriate referrals for access to a CCH. The CMC is responsible to monitor site vacancy, maintain waitlists, negotiate space offers with patients and families, and coordinate admissions to a CCH. The CMC is a liaison between the interdisciplinary team and service providers to support the patient and family to navigate the healthcare system. The CMC may address system, patient/family, and/or financial/legal delays to accessing a CCH through case management and referrals to healthcare professional and government or community programs. The CMC operates within provincial policy and procedures and maintains a patient centered approach in consideration of system resources.

Description:
As the Case Management Coordinator, you will be responsible for facilitating and coordinating client transfers. **Required Qualifications**:
Bachelor's degree in a health related field. Active or eligible for registration and practice permit with the applicable college.

**Additional Required Qualifications**:
Five (5) years relevant healthcare experience with one (1) year Seniors Health experience. Home Care, Seniors Health, dementia care, discharge planning, and experience with placement or working in DLO environment.
