Transition Coordinator

Reporting to the Aspen PCN Clinical Team Lead, the Transition Coordinator assists the patient and family through the transition to and from other health care settings, including to and from acute care settings, and assists with connecting patients and families with other health care teams or community programs as well as recommend non-health related community services.
**What you will do**:
- Assess patient needs in the context of the whole person including their life situation, determination of health and their family/supports teams
- Follow-up with patients discharge from acute or hospital care settings
- complete comprehensive needs assessments, determining urgency and priority
- Develop and implement appropriate service plans including providing information and assistance with referrals
- Coordinate service delivery across programs/settings, including liaising with community resources
- Provide information and support to clients, families, and caregivers to ensure they understand discharge/transition plans including identified goals, required follow-up appointments and any instructions provided
- Assist patients and families in seeking clarification when transition instructions are unclear
What you need to succeed
- Current CLPNA Registration in good standing and current CPR/BLS
- Critical Thinking and accurate interpretation of complex information from a variety of sources are essential skills for this position
- requires the ability to perform comprehensive assessments, planning and coordination of care, as well as ongoing communication with other health-care providers across the continuum of care
- Requires the ability to problem solve and cope with fluctuating priorities and time sensitive deadlines
- Conflict resolution and strong communication skills are required
- Demonstrates ability to look for innovative solutions when managing tasks.
- Strong oral, written and analytical skills: Self-directed and accountable
- Ability to maintain patient privacy and confidentiality
- Attention to details and problem solving skills
- Able to work independently and demonstrates ability to be proactive in completing duties
- Strong organizational and time management skills, including the ability to handle multiple assignments and task while meeting deadlines
- Ability to remain calm in difficult situations and deal effectively with those who are in distress
- Ability to maintain effective working relations with employees and customers
- Ability to collaborate with other multi-disciplinary team members
