We hire people that are skilled and passionate to meet our mission to end homelessness through outcome-based strategies which support personal and community transformation. The Housing Case Manager helps chronically homeless adults access and keep stable housing. This role is part of a team providing housing support, case management, and advocacy for clients in the CCC Shelter Plus Care program. Work includes helping clients find housing, navigate applications, build relationships with landlords, and connect with resources to maintain housing stability. This position provides direct behavioral and housing support to eligible participants of the CCC Shelter Plus Care program. Schedule: Monday - Friday, 8:00am-5:00pm Compensation: $23.69/hr Seniority Bid Window: Closes 12/31/25 MINIMUM QUALIFICATIONS: Must be registered with MHACBO to meet, at minimum, QMHA-R requirements. Bachelor’s degree in behavioral sciences field or a combination of at least three year’s work, education, training or experience in healthcare, chemical dependency, mental health treatment, or social services. Must possess a current driver’s license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage. Must meet CCC privileging requirements as required by FTCA. Must have current CPR certification prior to start date. Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit. Bi-lingual in Spanish preferred. Must have the ability to complete forms thoroughly, accurately and with legible handwriting. Must have a high tolerance for ambiguity and change. Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol. Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects. Able to climb stairs several times a day. Must be able to occasionally lift items of approximately 25 pounds. ESSENTIAL DUTIES: Conduct regular community outreach, including street outreach, to identify and locate eligible participants. Participate in regular team meetings and huddles. Schedule eligible clients for housing assessments, including MSST, and connect them to appropriate medical and behavioral health programs. Determine housing eligibility and coordinate placement in emergency shelter or transitional housing, or identify alternative housing options based on client needs and preferences. Enter and maintain accurate client data in HMIS. Coordinate with emergency shelter or transitional housing team members to support client placements and progress. Support clients in maintaining temporary placements through behavioral interventions, advocacy, and participation in care conferences. Engage regularly with clients to build housing readiness and independent living skills. Assist clients in achieving housing readiness, resolving legal issues, and gathering required documentation. Present housing opportunities through Shelter Plus Care and refer to alternative subsidy programs when SPC is not available. Assist with housing applications, intake, and placement coordination with CAA staff. Support voucher recipients with applications, appeals, and move-in processes, including arranging payment for fees, deposits, and rent when applicable. Coordinate inspections with property management and guide clients through the lease signing process. Arrange Community Warehouse services and help secure move-in items and funding. Provide ongoing support such as annual renewals, emergency assistance, landlord mediation, eviction prevention, and coordination during moves. Conduct regular outreach to clients’ homes and property management offices to ensure housing retention needs are met. Coordinate, track, and assist with the completion of yearly Home Forward renewal paperwork and inspections. Attend court hearings for criminal and eviction proceedings to support clients. Build and maintain professional relationships with landlords and community service providers. Offer continued support to SPC participants for the duration of their involvement in the program. Complete all paperwork and documentation in electronic health records accurately, per compliance requirements, and in a timely manner. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, andsecurity. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. Attend all mandatory CCC training in a timely manner. Perform other duties as assigned. SKILLS AND ABILITIES: Ability to work effectively in a fast-paced healthcare environment.
