You will be part of a diverse care team that will provide services to a roster of clients in the community. You willbe part of a collaborative team that will conduct joint care planning with a focus on client-centered goals, addressing theclient’s physical, cognitive, social, emotional, and spiritual domains, andcreating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate theinterdisciplinary team to reach theclient’s care goals. As part of the SE Transitions team, youwill be a part of the team that will be accountable for the care and services you provide and the outcomes your clients achieve. At SE, you meet your patients where they are, and together you take them to new heights RESPONSIBILITIES Conducting assessments Formulating goals and recommendations Planning and implementing client-centered, goal-directed treatment programs REQUIREMENTS A graduate of an accredited program in physiotherapy and is a member in good standing with the College of Physiotherapists of Ontario Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team Sound knowledge of community resources is an asset Effective interpersonal, organizational and communication skills A positive work and attendance record is essential Ability to adapt to new computer applications This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting A vehicle and valid driver’s license and the ability to travel About SE Health At SE, we love what we do.
