Would you like to be part of a team with a real community feel, which offers professional patient medical services from enviable practice premises located in the beautiful and charming coastal town and village of Whitby and Robin Hoods Bay?We are looking for a highly motivated, experienced Advanced Clinical Practitioner to join our well-established clinical team for up to 37.5 hours per week.You would be joining a friendly and dynamic practice team who serve a practice population of over 13,500 patients and aim to deliver modern primary care with the traditional feel of general practice.You will be supported by an excellent clinical team, a well structured management team and an outstanding administrative team.The practice RAG rates patients and we would expect our ACP's to see only green and amber patients in 15 minute appointments.Main duties of the jobIn this role you will work independently, making professional, autonomous decisions.The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care. Assessment may be face to face by appointment or by telephone as part of a triage/unscheduled care team and will include patients presenting with an undifferentiated diagnosis.You will be responsible for making any necessary referrals, requests for investigations and do these in in an appropriate manner and timescale.The role will involve independent prescribing within the scope of professional practice.The advanced clinical practitioner will support the GP team managing same day appointment requests from patients and home visits.The clinical practitioner will need to prioritise and triage the needs of patients accordingly, seeking GP input as appropriate where the problem lies outside their professional capability. They will demonstrate safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills.All appointments are 15 minutes.About usThe Practice is located in Whitby and has a branch site in Robin Hoods Bay. We have around 13,500 patients.Currently there are 3 partners supported by a team of several salaried GP's, Advanced Care Practitioners, several nurse prescribers and Practice Nurses, HCA's, Pharmacists, Pharmacy technicians, a dispensing team, care coordinators and a committed admin and management team.We are a training practice that works with Hull and York Medical School and currently support Year 4 medical students, GP Registrars and F2's.We have been rated Good by CQC and are a consistently high QOF achieving practice.Our clinicians have meetings built into the rota daily for half an hour to be able to discuss any ongoing cases and ask questions of colleagues.The practice is a very desirable place to work with a relaxed and pleasant atmosphere. It is very important to the partners that this is maintained and developed.There is a good social aspect to the team and we always have a Christmas party and a summer BBQ with several events in between.Job responsibilitiesClinical practiceAssess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosisClinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordinglyCarry out face to face and telephone, and triage patients appropriatelyAssess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needsProactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management planPrescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practiceWork with patients in order to support compliance with and adherence to prescribed treatmentsProvide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactionsPrioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency careSupport patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-careAssess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental HealthDelivering a quality serviceRecognise and work within own competence and professional code of conduct as regulated by the NMC/HCPCProduce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and proceduresPrioritise, organise and manage own workload in a manner that maintains and promotes qualityDeliver care according to NSF, NICE guidelines and evidence-based careAssess effectiveness of care delivery through self and peer review, benchmarking and formal evaluationInitiate and participate in the maintenance of quality governance systems and processes across the organisation and its activitiesUtilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where requiredIn partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriateEvaluate patients response to health care provision and the effectiveness of careSupport and participate in shared learning across the practice and wider organisationUse a structured framework (eg root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents and near-miss eventsAssess the impact of policy implementation on care deliveryContribute to the appraisal of the performance of the team, providing feedback as appropriateUnderstand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidanceWorking with practice management to ensure the whole team have skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour.
