Anticipated Start - End Date: ASAP to June 8, 2027 (subject to two weeks' notice) Reason for Term: Maternity Leave FTE: 1.0 Anticipated Shift: Days; Evenings Annual Base Hours: 2080 POSITION SUMMARY Social Workers provide client‑centred guidance and support to those being seen at Klinic. They uphold Klinic’s vision, mission and values while promoting social change, problem‑solving in human relationships, and the empowerment and liberation of people to enhance well‑being. Social Workers are part of the inter‑professional team at Klinic within Primary care including the Trans Health program. Taking into consideration the Social Determinants of Health model, Social Workers recognize the complexity of interactions between clients and their environment and the impact of this complexity on health and wellbeing of their clients. QUALIFICATIONS Bachelor of Social Work degree Current registration with the Manitoba College of Social Workers Minimum of two years supervised experience in community health or related area Experience in program and community development Ability to work in an inter‑professional environment Ability to work from a harm reduction model. Strong understanding of gender diversity and trans healthcare practices. Understanding the social determinants of health and their impact Demonstrated commitment to community‑based health care Knowledge of community resources Experience providing crisis counselling including risk assessments Experience facilitating groups Experience supporting social work student placements Awareness of and sensitive to issues of cultural diversity Ability to work from an anti‑oppression and anti‑racism perspective Demonstrated effective communication skills Excellent interpersonal skills Ability to work flexible hours, including evenings and weekends Ability to work independently and demonstrated decision‑making abilities Proven ability to work from an empowerment model Proven ability to optimize team and program functioning Knowledge of primary health care Proficient at Word, email, internet searches, and electronic medical records (Accuro an asset) RESPONSIBILITIES Provide social work services within the parameters of the Manitoba College of Social Workers Standards of Practice and Code of Ethics Conduct Trans health services client intake Provide short‑term supportive counselling Provide crisis support when a referral to the crisis lines is not appropriate Assist clients navigate formal social service systems such as EIA Provide support, information, referral and links to appropriate community resources Liaise with other programs at Klinic for health services Engage in building and supporting an inter‑professional environment Liaise with community resources/networks and service organizations Support Social Work student placements as a Field Instructor Facilitate groups Provide public education and training activities Provide support for learners Work as part of a team and promote team functioning Chart appropriately according to Klinic procedures Participate in training courses and services offered elsewhere and by Klinic to maintain and improve quality of service Attend staff meetings as required Participate in program planning, evaluation and quality monitoring including completing daily statistics Participate in formal self‑evaluations by identifying strengths and limitations Seek appropriate supervision/consultation of own actions Adhere to all agency policies, protocols, procedures and promote best practice Other duties and functions related to the job description not exceeding the above qualifications ADDITIONAL REQUIREMENTS Work environment consists of smudging Satisfactory Criminal Record, Child Abuse and Adult Abuse Registry checks SALARY RANGE $40.536 per hour - $52.900 per hour Klinic is committed to working towards having a more equitable and diverse workforce at all levels of our organization.
