We provide a wide variety of comprehensive care including primary care, dental, mental health and chronic disease management with a focus on illness prevention, health promotion and community development. PURPOSE We are currently recruiting for a Social Worker to provide clinical counselling and case management as a part of our Bloom team. Bloom Clinic is one of several provincially funded Hepatitis C outreach teams providing testing, treatment, and care to people who use drugs and people who are street involved across Peel Region. The successful candidate will have a strong foundation in harm reduction and work from an anti-racist, anti-oppressive, and trauma informed framework. RESPONSIBILITIES Provide counselling and case management to individuals living with, at risk of, and affected by HIV and HCV across Peel Region Support clients to develop goals and provide appropriate client-centred interventions Provide community-based education sessions on various issues including HIV, HCV, Harm Reduction, Mental Health, and more. Screen/assess for self-harm/risk factors and create safety plans with clients as needed. Engage in individual and systemic advocacy related to needs articulated by community members Determine need for additional supports and make appropriate external referrals as needed. Build and maintain strong relationships with community partners Maintain accurate and up to date client records in the EMR Ensure comprehensive treatment through ongoing consultation and collaboration with team members · Actively participate internal meetings as well as external community tables and committees QUALIFICATIONS, EXPERIENCE & SKILLS Master’s degree in Social Work or a related field Member in good standing with Ontario College of Social Workers and Social Service Workers and licensed to practice the controlled act of psychotherapy Strong experience with, understanding of, and commitment to harm reduction principles Demonstrated commitment to, and experience working from an anti-racist, anti-oppressive, and trauma informed framework Experience working with people experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, youth, and Indigenous community members Lived/living experience with any of the populations served by Bloom Clinic is an asset Experience working with communities and individuals who face barriers accessing traditional health care institutions such as poverty, discrimination, mental health, and substance use issues An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level Experience working with confidential and time sensitive documents and information Excellent interpersonal skills and the ability to multitask in a fast-paced environment Experience working in a value based non-profit or social services organization Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected Demonstrated proficiency in use of computers and various computer software applications, such as Electronic Medical Record (EMR) Ability to speak languages relevant to the residents of the organization’s community is an asset Travel is required for this role so candidate must possess a valid driver’s license and have access to a reliable vehicle with current insurance.
