POSITION SUMMARY:
The Hospitalist midwife supports a discharge planning and community visiting program within the Women’s Health and Newborn Care Program. Responsible for the day-to-day support of the program as well as support to the Birthing and Mother Baby Units in the care of low-risk obstetric and newborn patients. The role includes assessment, planning, teaching, newborn assessments and bloodwork, discharge timing and follow-up of obstetric and newborn patients.


RESPONSIBILITIES:

  1. Facilitates early discharge for dyads that meet the discharge criteria and arranges follow-up either in the hospital-based clinic or in the patient’s home.
  2. Performs weight checks, blood work on newborns, and postnatal obstetric care.
  3. Follows dyads for 7 days after discharge.
  4. Provides inpatient and outpatient newborn feeding support.
  5. Assists with patient navigation and connecting families with community supports.
  6. Performs NSTs.
  7. Attends Caesarean sections to promote skin-to-skin in the OR.
  8. Provides grief and bereavement support to families, including postnatal home visits.
  9. Support to the Early Pregnancy Assessment Clinic.
  10. Facilitates group visits and peer support groups for families.
  11. Conducts other duties as assigned by the Manager.

QUALIFICATIONS:

  1. Undergraduate degree in Midwifery required.
  2. Masters degree preferred.
  3. Regulated Health Professional with the College of Midwives of Ontario required.
  4. Professional membership with the Association of Ontario Midwives required.
  5. International Board Certified Lactation Consultant preferred.
  6. 3-5 years of clinical practice in midwifery, with current experience in a full scope hospital setting.
  7. Highly knowledgeable on professional midwifery standards and hospital guidelines and procedures.
  8. Exceptional interpersonal and communication skills with a proven track record of fostering collaborative partnerships.
  9. Committed to excellence in exceptional patient care and continual focus on quality improvement and results-driven outcomes.
  10. Demonstrate excellent critical thinking and problem-solving skills.
  11. Demonstrate ability to work in a dynamic fast-paced environment.
  12. Evidence of ongoing professional development, continuing education, and leadership experience.
