Our team is currently accepting applications for a Homecare Quality And Change Management Lead to join our team.Quality Improvement:

  1. Lead the identification, development, and monitoring of quality improvement initiatives that enhance client outcomes and team functioning.
  2. Compile and analyze homecare program data to support evidence-based decision making and identification of areas for improvement.
Change Management:
  1. Develop and execute change management strategies that support the successful adoption of new processes, technologies, and systems in the homecare program.
  2. Guide and mentor staff through transitions and practice changes, ensuring smooth implementation and minimized disruption to client care.
  3. Lead/co-lead staff education and training programs related to change initiatives, in conjunction with the Homecare Educator and other team members.
Policy and Procedure Development:
  1. Regularly review, revise, and implement clinical policies and procedures to ensure alignment with best practices and standards.
  2. Work with leadership to create new homecare policies as needed, ensuring that they support the organization's strategic goals and promote high-quality care.
  3. Ensure staff are trained on and compliant with all current policies and procedures.
Clinical Oversight:
  1. Provide clinical leadership and guidance to homecare nursing staff, ensuring the highest standards of client care are maintained.
  2. Conduct regular audits of clinical documentation and client care plans to ensure compliance with regulatory standards and best practices.
  3. Support staff development by identifying training needs and providing ongoing clinical education and mentorship.
Best Practices Implementation:
  1. Collaborate with interdisciplinary teams to identify and implement clinical best practices and homecare service delivery standards.
  2. Ensure client engagement remains a key priority within all change and quality improvement initiatives.
  3. Stay up-to-date with the latest trends, technologies, and evidence-based practices in homecare.
  4. Collaborate with external stakeholders, such as industry experts and regulatory agencies, to stay informed on best practices and emerging trends in home care.
Tasks:
  1. Monitors the quality of care provided to clients in areas of staff performance, responsiveness to clients, adherence to standards and legislative acts as required.
  2. Oversees the monitoring of any departmental and corporate safety/quality related indicators and processes for the Homecare business unit.
  3. Represents the Clinical Care component of Home Care at the management level and throughout the organization advising the Senior Director of risk management issues and solutions.
  4. Participates in corporate committees related to accreditation, incident investigation and abuse prevention, emergency planning and others related to quality and risk management.
  5. Leads the Homecare Quality Team, Change Management Team, any other CQI/audit committees and quality activities for Homecare, providing support and education to departmental staff as required.
  6. Promotes and facilitates staff involvement in quality and risk management activities.
  7. Coordinates/supports any investigations or reports as they pertain to incidents in Homecare.
Qualifications:
  1. Bachelors in Nursing
  2. Active registration with the College of Registered Nurses of Nova Scotia
  3. Valid driver's license and reliable vehicle
  4. Six to 10 Years of related work experience
  5. Experience in Homecare is an asset
  6. Experience in Project Planning and Project Management, Program/Policy Analysis and Development of Standards and Indicators
  7. Change Management experience/training
  8. Project Management experience
  9. Mental Health awareness training preferred
  10. Strong communication, interpersonal, and organizational skills
  11. Demonstrated skills in conflict resolution, problem solving, and time management
  12. Ability to maintain regular and consistent attendance

Hours of Work: 80 hours bi-weekly (8 hours) - Day

Salary/Benefits: As per Northwood Scale
