Job Description:

The Diabetes/Chronic Disease Educator is responsible for improving the quality of life of persons with chronic disease in the region through the provision of education, treatment and support. The Diabetes/Chronic Disease Educator serves as a resource to health-care professionals and others to ensure consistent, high-quality care for clients with chronic disease by sharing recent advances, knowledge and experiences of care. The Diabetes/Chronic Disease Educator reports to the Manager, Primary Health Care.

Experience Two years of recent nursing experience in a community based setting is required Education Diabetes Education certificate Bachelor of Science in Nursing (BScN) Other Education and Training Nursing diploma is acceptable Knowledge and Abilities Demonstrates effective verbal and written communication skills.
