In order to ensure that Milton District Hospital is able to remain a state-of-the-art healthcare facility, Milton District Hospital Foundation raises funds for essential medical equipment and technology required by healthcare providers to diagnose, treat and care for patients living in our community. All donations made to MDHF remain in Milton.

The Connected Care Halton Ontario Health Team (CCHOHT) is driving healthcare transformation by improving patient-centered, integrated care across primary, acute, home, and community settings. We are seeking a Lead, Integrated Population Health Initiatives to manage and execute high-impact projects that enhance care coordination, advance population health strategies, and improve health equity.

Reporting to the Executive Director, CCHOHT, this role will lead the development, implementation, and evaluation of integrated care initiatives, working closely with healthcare providers, system leaders, and community partners. The successful candidate will apply project management, stakeholder engagement, and change management expertise to support the delivery of innovative, patient-centered care models.

Key Responsibilities

  • Lead the planning, execution, and evaluation of multiple complex healthcare projects focused on integrated population health and system transformation.
  • Develop project roadmaps, set key milestones, and ensure deliverables align with the CCHOHT strategy and Ontario Health Team priorities.
  • Apply change management principles to drive adoption and sustainability of new models of care.
  • Track project performance, assess impact, and provide reports to stakeholders and leadership.

Stakeholder Engagement & System Coordination

  • Collaborate with healthcare leaders, primary care providers, specialists, and community partners to drive cross-sector alignment and engagement.
  • Foster relationships with patients, families, and caregivers, ensuring their voices shape program design and evaluation.
  • Work with Ontario Health, the Ministry of Health, and neighboring Ontario Health Teams to scale and spread best practices.
  • Facilitate working groups and advisory committees to support project implementation.
  • Identify opportunities to enhance population health, reduce health disparities, and improve health equity, particularly for marginalized communities.
  • Support the integration of digital health solutions and innovative care pathways to enhance patient experience and access.
  • Contribute to the development of policies, guidelines, and frameworks to support system-wide transformation.
  • Ensure alignment with the Quintuple Aim (population health, equity, patient experience, cost efficiency, and provider well-being).

Performance Measurement & Reporting

  • Monitor project progress using data-driven decision-making, identifying risks and mitigation strategies.
  • Prepare reports and presentations for leadership, funders, and government stakeholders.
  • Ensure compliance with health system policies, funding agreements, and performance measurement frameworks.

Education & Certifications

  • Post-secondary degree in Health/Life Sciences, Business Administration, Health Policy, or a related field (Master's degree preferred).
  • Project Management Professional (PMP) certification or equivalent is an asset.
  • Lean Six Sigma (Black Belt or Green Belt) certification is an asset.

Experience & Skills

  • 5+ years of progressive leadership experience in healthcare, system transformation, or population health initiatives.
  • Proven ability to manage complex, multi-partner projects with diverse stakeholders.
  • Strong understanding of healthcare integration, system design, and population health approaches.
  • Excellent stakeholder engagement and relationship-building skills, with the ability to work across organizational and sector boundaries.
  • Experience in change management, transformation leadership, and program evaluation.
  • Strong analytical, problem-solving, and decision-making skills with experience in data-driven performance measurement.
  • Exceptional communication skills, including report writing and presentation abilities.
  • Knowledge of Ontario's healthcare system and Ontario Health.
  • Hybrid role requiring a minimum of two days per week on-site at the OTMH hospital location.
  • Regional travel within CCHOHT communities (Oakville, Milton, Halton Hills) to engage with healthcare and community partners.
