Title: C74 - Program Navigator (Casual) Number of Positions: 2 Competition Number: C74 Location: Main Campus Position Status: Casual Reports to: Peer Coordinator & Senior Manager MHSU Compensation: $31.56 - $33.28 Job Summary As a member of the Phoenix Centre Mental Health and Substance Use Team, the Program Navigator, at Phoenix Society participates in the recovery and capacity building of residents to maximize their social and economic potential and support their goals for successful independent living in the community. Client Services Advocate for residents of the Phoenix Intensive Residential Treatment Program (IRT) Discharge planning services for those residents who have participated in IRT programming. i.e.: housing search in home community, resource and referrals. Ensure transportation for IRT clients arriving to and leaving from the Phoenix Center at intake and discharge Assist in acquiring personal documentation Ensure ongoing assessment of client progress regarding levels of support needed Support individuals through crisis including tenancy breakdown Provide support for clients to develop life skills and support community engagement Offer assistance for rental applications as well as other required documentation such as applying for personal identification, healthcare requests, training, jobs, etc. Educate and provide consultation to clients, families and healthcare professionals Make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community partners Develop, coordinate and deliver psycho-educational and support groups Ensure that client files are maintained and current within established guidelines Report to and discuss case concerns with supervisory positions and the integrated services team Maintain accurate statistics throughout practice and provide monthly reports Create and maintain a therapeutic environment following therapeutic community principles which encourage awareness of inter-dependence of community members, and the value of responsible concern for self and others Act as a positive role model for staff and clients Act as a back-up to outreach or support staff as needed Operations Identify areas for improvement and increased efficiency in building and program operations, technology, services offered, reporting and reports to supervisory positions Follow and implement universal precautions and infection control Ensure that program regulations, policies and procedures are followed Maintain up-to-date knowledge of internal safety policies and procedures, as well as other emergency procedures Organize and handle client information in accordance with all privacy legislation Organize and assemble monthly administrative reports Maintain agency standards of work quality Community Relations Develop positive collaborative relationships with appropriate community partners and external stakeholders Represent the Agency in the community through effective and professional interaction with all stakeholders (e.g., BC Housing, Fraser Health, BC Corrections Branch, etc.), human service and health service agencies, government, media, neighbours and the general public Maintain a thorough knowledge of social, economic, recreational and educational resources in the community Attend public and organizational events as required Foster teamwork within the Phoenix Society and with service stakeholders. Take action required to foster teamwork and resolve problems that may arise Qualifications Post‑secondary diploma (e.g., 2 to 3 years) in community social services. Minimum one (1) year of experience working with marginalized populations, substance use services, mental health, corrections, etc., or equivalent Ability to administer Narcan and respond effectively to overdose.
