Program Director - Community Integration

Permanent Full- Time

Reporting to the Associate VP Continuing Care & Community Integration, the Program Director is responsible for all activities related to the administrative and patient care functions within the designated portfolio. The Program Director will work with Clinical and Program leaders and Community and Government Partners to ensure activities and programs are consistent with Mackenzie Health (MH) and the Western York Region-Ontario Health Team (WYR-OHT) goals/objectives and that efficient/effective administrative and clinical practices are provided.

The portfolio will include oversight of the Mackenzie Helps @Home Program, Transitional Care Unit, Mackenzie Health Long Term Care Home, Nursing Led Outreach team (NLOT). This portfolio also includes the ever-growing operations of the WYR-OHT as it progresses towards maturity and expands to include new functions of the WYR-OHT such as assuming Home and Community Care contract oversight and Primary Care Network expansion.

The Program Director is responsible for the successful leadership and overall management of the WYR-OHT according to the priorities determined by the Collaboration Council. The Program Director will be guided by the shared vision, guiding principles and strategic plan of the WYR-OHT, including the principles of population health management, health equity, patient, family and caregiver partnership and engagement. The Program Director is also responsible for collaborating with leadership to ensure effective co-ordination of efforts for optimal patient care; providing exceptional and visionary leadership in the development of the program, organizational and financial plans; liaising and collaborating with internal and external professionals and stakeholders to ensure practice standards are aligned and integrated across and within the Program(s); ensuring achievement of the strategic and operational plans; representing the Program globally as an expert in community integration through sound and influential decision-making; performing other duties consistent to the job classification.

PRIMARY RESPONSIBILITIES

  • Responsible for the overall organization and leadership of the programs and services within the portfolio, including patient care delivery, operational planning, Human Resource development, financial management, quality management and education and research.
  • Provides leadership in the definition of strategic direction and ensures excellence in patient care and service delivery through continuous quality improvement processes that include planning, quality monitoring and risk avoidance to the patients and the organization.
  • Drives accountability to meeting the Transfer Payment Agreement deliverables as set out by Ontario Health.
  • Participates as a member of the Program Director Team, facilitating program planning, goal setting, evaluation and assisting in the management of the fiscal, human, and material resources assigned to the portfolio, as well as establishing standards and development and delivery of program to patients and clients.
  • Develop and implement structures, processes, and tools to enable integrated delivery of results.
  • Develop effective and collaborative working relationships with the members of the Collaboration Council, the WYR-OHT partners, Ontario Health, the Ministry of Health and relevant internal/external stakeholders to ensure the continuity of relationships, information exchange, and for planning, implementation and operations.
  • Fosters partner engagement and nurtures and supports change management, while guiding his/her teams through change.
  • Continuously support the clinical operations plan and the achievement of the goals within it by developing, implementing, and overseeing operations planning functions to formulate goals in alignment with organizational activity. This includes facilitating alignment across leaders and the organization by leading cross-functional multi-year and regular planning/refresh processes.
  • Develop effective and collaborative working relationships with the members of the leadership team, relevant internal/external stakeholders (including Patient Partners and other external advisory councils), and staff across major projects to ensure the continuity of relationships, information exchange, and sounding boards for new initiatives.
  • Lead and facilitate planning activities, including the design of integrated governance structures as required. This involves co-leading select clinical initiatives and may include establishing and overseeing the necessary resources and teams (internal and external).
  • Shares Hospital Administrative On Call Responsibility and assumes responsibility for the Directors of other programs and services in their absence as requested.
  • Perform other duties as required.

QUALIFICATIONS

  • Regulated Health Professional with master’s level preparation in Health or Business Administration or equivalent.
  • Management experience of at least 5-8 years in middle level management in hospital or community environment coupled with a strong clinical background in primary care, chronic disease management and population health.
  • Previous experience at the Director level preferred.
  • Progressive leadership experience in quality, resource and utilization management and planning, effective communication, and achievement.
  • Demonstrated significant experience in a leadership role.

  • Experience with strategy development and execution.
  • Experience with developing and managing budgeting and reporting.
  • Experience in partnership development, including engagement and partnership contracts, an asset.
  • Superior financial and statistical management abilities; including the ability to assess, interpret and project budgetary needs.
  • Ability to work with all health disciplines to achieve best practices.
  • Possess team building, creative thinking, and ability to function effectively during periods of rapid change and transition.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Excellent interpersonal, customer service, analytical, teamwork, leadership skills and conflict resolution skills.
  • Valid driver’s license and reliable vehicle to support regular travel between all three sites: Reactivation Care Centre, Mackenzie Richmond Hill Hospital and Cortellucci Vaughan Hospital
  • Ability to perform the essential duties of the job.

WORK ENVIRONMENT

  • This position is an on-site position.
  • Potential need to travel throughout Western York Region.
  • You may be required to work at any site of Mackenzie Health
