POSITION DESCRIPTIONJOB OVERVIEW
  • We are seeking a Licensed Practical Nurse to join our Community Health Centre Team at CNH. The position provides primary care, health promotion and outreach nursing services as part of an interdisciplinary team.
  • Practice within the context of a client and family centered care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for Licensed Practical Nurses and within RISE policies, guidelines and clinical practice documents.
  • Work within an interdisciplinary team-based care model of Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dieticians, Physio Therapists, Counsellors and Community Health Workers to assess, plan implement and evaluate and document client care and collaborate with other Community Care Services.
  • There will be opportunities in this role for case management; wound management; administration of medications and immunizations; performing phlebotomy; performing mental health and addiction care; and chronic and infectious disease management.

Skills and Qualifications

  • Graduation from a recognized program for Practical Nurses.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years recent experience working as a Licensed Practical Nurse providing care through a holistic mental health, substance use and primary care lens or an equivalent combination of training, education and experience.
  • Knowledge of Community Health Nurses Standards of Practice and Guidelines for clinical practitioners.
  • An egalitarian philosophy and non-judgmental attitude (i.e. gender, culture, race, sexual orientation, language, ability and more).
  • Previous experience providing culturally safe, trauma-informed care to marginalized populations.
  • Training and demonstrated ability to provide nursing assessments and interventions related to common diagnosis and specific clinical situations relevant to vulnerable populations (e.g. chronic disease, infectious disease, chronic pain, addictions, mental health).
  • Demonstrated ability to perform focused secondary assessments, integrate information from a variety of sources including diagnostics, and develop plan of care relevant to the priority populations.
  • Demonstrated ability to employ teaching strategies to transfer knowledge to engage clients, families and groups in planning and self-management of care.
  • Experience and comfort drawing blood.
  • Completion of BCCDC Online Immunization Competency Course for Licensed Practical Nurses or willingness to achieve the same shortly after starting position.
  • Demonstrated ability to provide treatment planning, counseling, crisis intervention, and case coordination.
  • Demonstrated ability to communicate effectively, both orally and in writing, one-on-one and in group settings, with colleagues, clients and their families.
  • Demonstrated ability to provide health teaching in a group setting.
  • Demonstrated ability to effectively employ conflict resolution and reconciliation techniques and approaches.
  • Experience promoting collaboration, capacity building, teamwork, and communication within diverse interdisciplinary teams.
  • Experience applying strategies or practices to address social determinants of health and health inequity.
  • Broad knowledge of crisis intervention and counseling skills.
  • Broad knowledge of mental health issues and substance use and treatment, including harm reduction approaches.
  • Broad knowledge of local community health and social service resources and referral options.
  • Excellent planning, prioritization and time management skills.
  • Ability to utilize Electronic Medical Records, word processing, Internet and email software.
  • Ability to provide services through outreach.
  • Physical ability to perform the duties of the position.

Additional assets include:

  • Additional nursing courses, certifications, or experience in: phlebotomy, immunization, addictions care and wound care.
  • Fluency in identified non-official languages based on neighborhood demographics.
  • Experience working with medical interpreters.

Salary: $32.84–$41.35/hr

Benefits: Extended health and dental benefits; Multi-sector pension plan; Annual reimbursement for professional dues (prorated to FTE); Paid vacation starting at 4 weeks per year (prorated for FTE) plus paid sick days.

Schedule: 16.5 Hours per week (0.44 FTE), Friday 9-6:30pm & Saturday, 9:00-5:00pm, half-hour unpaid lunch breaks.

Location: RISE Community Health Centre, 5198 Joyce Street, Vancouver and community settings in Renfrew Collingwood.

Expected Start Date: ASAP for training and orientation.
