The Opportunity
Reporting to the Options Program Manager, the Developmental Services Triage Case Manager (DS-TCM) provides triage to all referrals received from Developmental Service Ontario – Toronto (DSO-TR), conducts intake, determines service pathways, makes referral to appropriate program and services, manages waitlist and referrals in an electronic client database(s). Provides goal focused time-limited service to a case load of clients with developmental disabilities and their families/caregivers. Develops and reviews individual service plans regularly with clients and their families/caregivers. Provides information to help clients make informed choices; to access both funded and non-funded community resources. The DS-TCM uses an individualized and person-directed approach, focusing on the client’s strengths, facilitating inclusion and promoting skill and knowledge development.
Responsibilities
- Triage and assess clients’ referrals received from Developmental Service Ontario - Toronto (DSO-TR) for case management support.
- Actively maintain and manage the DS case management referral and waitlist for Toronto region.
- Inputs client data within electronic client record databases
- Refer clients to appropriate internal and external resources in the DS case management system/model.
- Respond to clients, families/caregivers, service partners queries regarding case management model processes and procedures.
- Assists clients in identifying their strengths, needs and goals using a standardized intake triage tool
- Determine service streams (short or long term) and service pathways within the Centralized Case Management model.
- Collaborates with DSO-TR, service providers and community resources as needed.
- Carry a caseload of clients for purposes of providing case management supports
- Advocates with the client/family for appropriate resources as needed.
- Monitors the implementation of the individualized service plans and review effectiveness with the client/family on a regular basis and ensures continuity of service while in receipt of services within the case management model.
- Facilitates group education sessions.
- Strong documentation skills.
- Attends and participates in team meetings.
- Attends regular supervision with the manager.
- Identifies and reports high risk situations and serious occurrences as required.
- Ensures the regular and timely recording of case notes, client and service data as required by funders and FST policy.
- Attends case consultation meetings, training and staff development opportunities as required.
- Implements/participates in program evaluation and client record audits.
- Delivers service and conducts work in accordance with the Quality Assurance Measures and accreditation requirements.
- Represents FST at community planning/partnership tables as requested.
- Supervises students and/or volunteers.
- Performs other assigned duties as required.
Working Conditions
- Travel is required in and around Toronto
- Requirement for flexibility in schedule as required
- Requirement to work alone in the community and in client’s home
Qualifications
- Post-secondary degree in social work or a related field of study or equivalent combination of education and experience
- Minimum of three years of clinical/case management experience working with individuals with intellectual/developmental disabilities and families.
- Knowledge of referral processes within the developmental services sector
- Experience developing short- and long-term individualized service plans at the intake/assessment level
- Knowledge and experience providing service and supports to those diagnosed with a Dual Diagnosis (i.e. Mental Health and Developmental Disability).
- Knowledge of evidence based practices and supports for individuals with developmental/intellectual disabilities.
- Knowledge of trauma informed approach to service delivery.
- Demonstrated assessment, formulation, planning, intervention and evaluation skills.
- Experience facilitating group education sessions.
- Demonstrated knowledge of developmental services and community resources in Toronto.
- Demonstrated understanding of the issues faced by people with intellectual/developmental disabilities.
- Demonstrated commitment to person-directed service.
- Strong advocacy skills.
- Demonstrated ability to engage in creative problem solving and decision making.
- Demonstrated ability to foster independence in others.
- Strong interpersonal and communication (verbal, written and listening) skills.
- Strong organizational, time management and computer skills (familiarity with database applications).
- Strong conflict resolution skills, diplomacy and tact when dealing with challenging situations.
- Demonstrated ability to work in a team and build strong relationships with service partners.
- Demonstrated ability to effectively collaborate within the developmental service sector, as well as across sectors, for the supports clients need and to build capacity of others.
- Ability to work collaboratively and to develop strong professional relationships with internal and external partners.
- Demonstrated commitment to principles of social justice, anti-oppression, equity and inclusion.
- Ability to work flexible hours as required.
- French or another second language is an asset.
- Valid First Aid and CPR and CPI considered an asset.
- Satisfactory Criminal Reference Check with Vulnerable Sector Check.
