Job Summary

Fraser Health is seeking a skilled Registered Nurse to join our Cardiac Services team. As a key member of our interprofessional care team, you will provide direct patient care by assessing, planning, providing, and evaluating nursing care to patients in an acute medical unit.

Responsibilities
  1. Conduct thorough patient assessments, collecting information from various sources, and making nursing diagnoses to determine the best course of care.
  2. Collaborate with patients and their families/caregivers to develop and implement individualized care plans, promoting patient autonomy and dignity.
  3. Participate in the care and discharge planning process, ensuring comprehensive care planning and delivery, and communicating effectively with the interprofessional care team.
  4. Coordinate nursing care, prioritizing tasks, delegating to support staff, and making referrals to the interprofessional care team as needed.
  5. Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to support self-care and discharge planning.
  6. Document assessments, nursing diagnoses, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
  7. Participate in nursing committees, care conferences, and councils as assigned or agreed upon with the Manager.
  8. Contribute to research and quality improvement activities by identifying patient care issues, collecting data, and recommending corrective actions and changes to practices, procedures, or protocols.
  9. Participate in in-services and educational programs, maintaining current clinical competence and knowledge within the area of practice.
  10. Act as a preceptor or mentor to new nursing staff and students, sharing knowledge and demonstrating care activities.
  11. Provide feedback to the Manager/delegate regarding the effectiveness of patient care programs, policy/clinical decision support tools, and equipment.
Qualifications

Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Completion of Cardiology component of Post Basic Critical Care course, or post basic High Acuity Course, approved Arrhythmia interpretation course, plus one year's recent experience in cardiac monitoring or telemetry unit, or an equivalent combination of education, training, and experience. Current Basic Rescuer Certificate.

Skills and Abilities:

  • Clinical proficiency in the designated clinical area.
  • Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety type interventions.
  • Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team using verbal, written, and electronic communication means.
  • Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
  • Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being.
  • Ability to organize and prioritize work.
  • Ability to collaborate with and promote cooperation among members of the interprofessional care team.
  • Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
  • Ability to operate related equipment, including relevant computer applications.
  • Physical ability to perform the duties of the position.
