Job Summary

We are seeking a highly skilled Registered Nurse to join our Cardiac Services team at Fraser Health. As a key member of our interprofessional care team, you will provide direct patient care, assess, plan, provide, and evaluate nursing care in an outpatient clinic setting.

Key Responsibilities
  • Provide preadmission screening of surgical patients to ensure they can proceed with scheduled surgery
  • Assess patients referred to designated clinical outpatient clinics, collect information from various sources, and make nursing diagnoses
  • Plan, perform, and evaluate nursing interventions and care, coordinate nursing care for patients, and make referrals to the interprofessional care team as needed
  • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures
  • Teach patients, families/caregivers about risk reduction strategies, emergency management, use of prescribed medications, self-management skills, lifestyle adjustments, and health maintenance
  • Collaborate with patients and their families/caregivers in developing care plans, encourage family participation, and establish a therapeutic relationship
  • Participate in care and discharge planning, ensure comprehensive care planning and delivery, and collaborate with the interprofessional care team
Requirements
  • Graduation from an approved Nursing Program recognized by the Canadian Nurses' Association with current practicing registration as a British Columbia College of Nurses and Midwives (BCCNM)
  • Two years' recent, related experience in the education, monitoring, and management of participants in cardiac prevention and rehabilitation, including exercise training and experience with ECG Telemetry monitoring
  • ECG rhythm interpretation course completion or eligibility
  • Current Basic Cardiac Life Support (BCLS) Certification
  • Current Advanced Cardiac Life Support (ACLS) Certification
  • Valid BC Driver's License
Skills and Abilities
  • Clinical proficiency in the designated clinical area
  • Ability to complete initial and ongoing patient assessment and apply nursing theory and expertise to provide nursing care
  • Ability to recognize acuity changes and use factual information to support nursing decisions and actions
  • Ability to communicate effectively with patients, families/caregivers, and the interprofessional care team
  • Ability to promote person-centered care and integrate and evaluate data from multiple sources to problem-solve effectively
