We are currently looking for a Surgical Care Coordinator (SCC) for our Toronto site.

As an integral member of Altum Health, the Surgical Care Coordinator (SCC) co-ordinates, schedules and organizes client surgical and post-surgical activities. The SCC role is one of client ambassador and acts as a liaison to relay information to the client health care providers, UHN and Altum staff, WSIB and other key stakeholders in a timely manner. By creating this channel of communication, the SCC is able streamline the client's care and minimizes anxiety and confusion related to the surgical experience. As such, the SCC becomes the face of Altum Health for our clients from surgical consent to post-surgical rehabilitation, providing friendly information; answering questions and helping our clients navigate seamlessly through what can be a complicated system. The SCC demonstrates the ability to build, maintain and enhance relationships with all stakeholders, problem solve within a team environment as well as demonstrate excellence in customer service with each and every interaction.

The SCC also handles a myriad of detailed responsibilities pre- and post-surgery to facilitate program efficiency. This includes information system data entry, preparing client charts and letters, ensuring all appointments are scheduled and all client tests are completed as required, surgical supplies are ordered and available when needed, and in-patient resources are arranged as necessary. Demonstrated track record of client and customer service is required.

Qualifications
  • Completion of a College Diploma or University Degree required
  • Healthcare background/knowledge is an asset
  • Excellent written and verbal communication skills
  • Excellent decision-making and problem-solving skills
  • Excellent time management and organization skills
  • Able to direct emergency client needs
  • Capacity to work flexible hours Monday to Friday
  • Capability to function collaboratively within a multi-disciplinary team and/or independently
  • Ability to work in a self-directed manner
  • Ability to multi-task and work in a fast-paced and changing environment
  • Ability to liaise across various health departments to provide solutions and excellent client centered care, including establishing and managing relationships to foster effective collaboration
  • Demonstrated awareness of and adherence to professional and ethical standards
  • Computer literacy
  • Experience working with WSIB is an asset
  • Knowledge of all Altum Health product lines is an asset
