About the Role

We are seeking a highly skilled Licensed Practical Nurse to join our team as an Outreach Specialist. In this role, you will work as a member of a multidisciplinary mental health services team, providing personal and nursing care services to clients in their homes and in the community.

Key Responsibilities
  • Administer medications in accordance with pharmaceutical and organizational policies and procedures.
  • Assess and monitor clients' physical, psychological, socio-cultural, and spiritual needs on an ongoing basis.
  • Provide a supportive environment for clients, contribute to the development and modification of client care plans, and attend interdisciplinary care planning meetings.
  • Plan, organize, and establish client care plan priorities when providing care, responding to unexpected events and changing client needs within scope of practice and competency guidelines.
  • Provide nursing care to clients according to identified policies and procedures, technology, and Fraser Health standards, evaluating outcomes, recording observations, and reporting problems and/or changes to designated staff.
  • Document and update relevant care and procedures information to ensure patient safety.
  • Assist clients with activities of daily living, such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs, and oversee client exercise routines as applicable.
  • Participate in quality improvement and program activities to maintain a consistent level of care and ensure that interdisciplinary team members work collaboratively.
  • Maintain a safe working environment for clients and others by adhering to established safety policies, reporting unsafe conditions and/or malfunctioning equipment.
  • Assist in the orientation and education of new staff, students, and other members of the interdisciplinary team, in collaboration with the Manager or designate.
Requirements
  • Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training, and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Current First Aid, CPR, and Food Safe certificates.
  • Completion of or eligible to complete MHSU Competency Education.
  • Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
  • Ability to communicate effectively, both verbally and in writing.
  • Physical ability to carry out the duties of the position.
  • Ability to work independently and in cooperation with others.
  • Ability to operate related equipment.
  • Ability to organize and prioritize.
  • Ability to establish and maintain rapport with clients.
  • Ability to observe and recognize changes in client condition.
