Job Title: Clinical Social Worker About the Role: We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health. As a Clinical Social Worker, you will provide clinical social work counseling services to clients and families with serious, persistent mental illness and substance use in the Assertive Community Treatment (ACT) program. Job Summary: The successful candidate will provide comprehensive social work and psychosocial assessments of clients with mental illness and substance misuse/dependence, and families. They will also educate and provide consultation to clients, families, and healthcare professionals on issues related to high risk factors, recovery, and current community practice in the field of mental health. Responsibilities: • Provides comprehensive therapeutic clinical counseling services to clients with mental illness and their families, community supports, and interdisciplinary treatment teams to coordinate services, manage cases, and facilitate meetings. • Conducts comprehensive social work and psychosocial assessments of clients with mental illness and substance misuse/dependence, and families by interviewing the client and/or family, obtaining relevant information, gathering social data regarding the client and/or family, and interpreting the data using a variety of modalities. • Educates and provides consultation and support to clients, families, and healthcare professionals on issues related to high risk factors, recovery, and current community practice in the field of mental health. • Maintains client records by documenting client assessments, charting client information, preparing progress notes and reports in accordance with established standards, policies, and procedures. • Arranges meetings to develop and coordinate care plans and community services for clients requiring facility placement and/or community support and collaborates with hospital and community services from a social work perspective to determine appropriate levels of care and the effective utilization of client resources. • Communicates/collaborates with mental health and related community service agencies and family physicians by collaborating in the assessment and management of individuals at risk, making referrals to appropriate resources when longer-term treatment or follow-up is required, and exchanging relevant information. Qualifications: • Masters Degree in Social Work (MSW) from an accredited school of Social Work. • Two (2) years' recent, related experience in a mental health treatment setting or an equivalent combination of education, training, and experience. • Current full registration with the BC College of Social Workers. • Valid Class 5 BC driver's license with access to personal vehicle for business purposes. Skills and Abilities: • Comprehensive knowledge and understanding of emotional issues related to mental illness and addictions and client and family cultural dynamics. • Comprehensive counseling skills including crisis counseling, brief therapeutic interventions, and group work applicable to clients and families. • Demonstrated ability to organize and prioritize workload in a complex environment. • Ability to educate and provide consultation to clients, families, and healthcare professionals. • Ability to provide leadership to the interdisciplinary team in care planning for the effective utilization of client resources. • Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with community health and social agency personnel.
