Clinical Social Worker, Primary Care Specialist

We are seeking a highly skilled Clinical Social Worker to join our Primary Care team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive social work and psychosocial assessments, develop and implement treatment plans, and collaborate with healthcare professionals to deliver high-quality patient care.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments of clients and their families, using a variety of assessment tools and techniques.
- Develop and implement treatment plans, including short-term brief/crisis intervention for patients with mental health and substance use issues.
- Collaborate with the interdisciplinary team to coordinate services, manage cases, and facilitate meetings.
- Provide education and consultation to patients, families, and healthcare professionals on relevant health care and privacy legislations.
- Participate in the admission and discharge process of clients in acute care and community, and arrange meetings to assist in developing and coordinating comprehensive care plans and community services.
- Act as a clinical resource for clients and their families, providing information regarding community resources and access to these resources.
- Maintain patient records, documenting assessments, charting information, and preparing progress notes and reports in accordance with established standards, policies, procedures, and current regulatory requirements.
- Current full registration with the BC College of Social Workers.
- Two years' recent related experience in a complex healthcare environment, such as a community or home health setting.
- Valid B.C. Driver's license and access to a vehicle with appropriate business coverage.
- Comprehensive knowledge and understanding of counseling and clinical practice models as it relates to individuals, families, and groups.
- Demonstrated ability to conduct comprehensive social work and psychosocial assessments.
- Comprehensive counseling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to patients, families, and their support networks.
- Ability to educate and provide consultation to patients, families, and healthcare professionals.
- Ability to effectively work independently and with others, including working effectively in a multicultural environment, as part of an interdisciplinary team.
