We are seeking a highly skilled and compassionate Social Worker I to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide critical support to patients and families, helping them navigate complex healthcare systems and addressing their psychosocial needs.
Key Responsibilities- Conduct comprehensive psychosocial assessments to identify patient and family needs, and develop personalized treatment plans.
- Provide clinical and supportive counseling services to patients and families, addressing issues such as adjustment to illness, personal concerns, and lifestyle changes.
- Collaborate with healthcare teams to provide crisis intervention, crisis resolution, and support services.
- Connect patients and families with community and rehabilitation resources, and provide education on accessing these services.
- Participate in quality improvement activities, contributing to the development and evaluation of departmental and program goals.
- Master's degree in Social Work from an approved school of Social Work, plus two years' recent, related clinical experience.
- Current full registration with the BC College of Social Workers.
- Comprehensive knowledge of systems theory, social work assessment and intervention, and developmental stages in the individual and family life cycle.
- Ability to establish rapport with clients and family members, and communicate effectively both verbally and in writing.
