Job Summary: Care Coordinator for Continuing Care at Nova Scotia Health, responsible for assessment, care planning, and case management of clients in the community. The ideal candidate has a background in nursing, social work, or a related field, with experience in community or long-term care settings.

About the Role:

  • Assess clients' needs and develop care plans to support their goals and independence.
  • Authorize services and make referrals to community resources to support clients in their homes.
  • Work with interprofessional teams to facilitate hospital discharge and return to the community.
  • Provide ongoing communication and support to clients, families, caregivers, and healthcare teams.

About You:

  • Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
  • Current registration with the applicable Nova Scotia Professional Regulatory body.
  • Minimum three years' experience in community or long-term care settings.
  • Knowledge of community agencies and resources.
  • Excellent assessment, interviewing, and negotiation skills.
  • Ability to work independently and collaboratively in a busy multidisciplinary environment.

Hours and Compensation:

  • Permanent Full-time / 70 hours bi-weekly.
  • Shifts may include days, evenings, weekends, and holidays.
  • $41.06 - $46.75 Hourly.
