About the Opportunity

We are seeking a skilled and experienced Continuing Care Coordinator to join our team as a Team Leader. In this role, you will provide leadership, innovation, and support with assessment, authorization, service planning, and collaborative case management aligned with provincial policies for continuing care programs and services.

You will work closely with continuing care staff and managers, contracted home support, nursing and long-term care service providers, NSH clinical programs and team members, primary healthcare providers, and community agencies and organizations to promote person-centred care.

The ideal candidate will have a Bachelor's degree in Social Work, Occupational Therapy, Physiotherapy, Nutritional Studies, or Nursing, and a minimum of five years of related experience. They will also have demonstrated leadership capabilities, the ability to conduct comprehensive multi-dimensional assessments, and experience developing, monitoring, and evaluating complex care plans.

The Team Leader will be responsible for fostering healthy team processes, facilitating collaboration, and participating in program planning and evaluation. They will also assist in the orientation and mentorship of new team members and support a quality approach to service delivery.
