Continuing Care Coordinator

The Continuing Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. They work with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Coordinator is fiscally responsible for the care, services, and equipment they authorize within available Department of Seniors and Long Term Care policies and programs.
The ideal candidate will have a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. They will have a minimum of three years' experience in a community or long-term care setting, with knowledge of community agencies and resources.
The Coordinator will work independently and cooperatively in a busy multidisciplinary situation, with excellent assessment, interviewing, and negotiation skills. They will be able to work with clients, family, caregivers, care providers, and healthcare teams to identify goals, unmet needs, and resource requirements.
The Continuing Care Coordinator will be responsible for reassessments at routine intervals and skilled care management. They will work with clients to articulate and advocate for alternative solutions to address unmet needs.

The ideal candidate will have a valid driver's license and access to a reliable vehicle. They will be able to work in a fast-paced environment, with excellent time management and prioritization skills.
HoursThe Continuing Care Coordinator will work 70 hours bi-weekly, with shifts that may include days, evenings, weekends, and holidays.
