Job SummaryAs a Registered Nurse with Fraser Health, you will be responsible for delivering person-centered care to patients in an acute medical unit. You will work collaboratively with the interprofessional care team to assess, plan, provide, and evaluate nursing care to patients.
Responsibilities- Assess patients and collect information from various sources to make informed nursing diagnoses and develop effective care plans.
- Collaborate with patients and their families/caregivers to develop and implement care plans that promote patient autonomy and dignity.
- Participate in the care and discharge planning process, ensuring comprehensive care planning and delivery.
- Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff as needed.
- Collaborate with the interprofessional care team to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
- Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to support patient self-care and discharge planning.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Participate in research and quality improvement activities, identifying patient care issues, collecting data, and recommending corrective actions.
- Participate in in-services and educational programs to maintain and update current clinical competence and knowledge.
- Act as a preceptor or mentor to new nursing staff and students, sharing information and demonstrating care activities.
Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Completion of applicable post-graduate coursework or an equivalent combination of education, training, and experience.
