Responsibilities- Conduct thorough patient assessments, identifying and prioritizing patient needs to deliver timely and effective care.
- Develop and implement individualized care plans, collaborating with patients and their families to ensure informed decision-making and optimal patient outcomes.
- Collaborate with the interprofessional care team to ensure comprehensive care planning and delivery, including coordination of care and discharge planning.
- Provide education and support to patients and families, promoting health literacy and self-care skills to empower patients to manage their health effectively.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices to enhance patient care and outcomes.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of applicable post-graduate coursework or an equivalent combination of education, training, and experience.
- Current certification in Basic Cardiac Life Support (BCLS).
