Job Title: Patient Care Coordinator Job Summary: We are seeking a highly skilled Patient Care Coordinator to provide clinical supervision, guidance, and support to nursing staff and other clinical team members in a rapidly growing organization. Job Description: The Patient Care Coordinator is responsible for the day-to-day operations of the assigned unit(s), including daily patient flow, patient discharge, and patient placement/referral. The successful candidate will have a strong clinical background, excellent leadership and communication skills, and the ability to prioritize tasks and manage multiple responsibilities. Key Responsibilities: • Provides clinical supervision, guidance, and support to nursing staff and other clinical team members • Coordinates patient assignments, monitors staff workload, and adjusts patient assignments accordingly • Establishes priorities for patient care, patient flow/discharge planning, and overall daily operations of the designated unit(s) • Monitors the quality of patient care and ensures maintenance of standards for nursing practice and nursing support staff • Provides input into the development of learning objectives, provides clinical support and guidance, and evaluates the effectiveness of learning opportunities • Acts as a point person for patients and families who have concerns or questions regarding their care • Participates in quality improvement and risk management activities Qualifications: • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) • Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience • Completion of post-graduate coursework and/or certification applicable to the designated clinical area Skills and Abilities: • Clinical proficiency/expertise in the designated clinical area • Ability to supervise and provide leadership and work direction • Ability to coordinate patient assignments, in collaboration with staff • Ability to monitor staff workload and patient factors and adjust patient assignments accordingly • Ability to establish priorities for patient care, patient flow/discharge planning, and overall daily operations of the designated unit(s) • Ability to monitor the quality of patient care and ensure maintenance of standards for nursing practice and nursing support staff About Fraser Health: Fraser Health is a leading healthcare organization in British Columbia, Canada, serving over two million people in Metro Vancouver and the Fraser Valley.
