Our Care Coordinators are the heart of our organization, working collaboratively with patients and their families to develop individual care and service plans. You will link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.

We're looking for someone with a strong background in community health or a related field, who is passionate about delivering patient-centered care. If you have excellent interpersonal, communication, assessment, problem-solving, and decision-making skills, we want to hear from you.

Key Responsibilities:

  • Assess patient care needs and determine eligibility for services
  • Link patients with service providers
  • Coordinate and monitor care plan delivery
  • Establish a helping relationship with patients and their families

Requirements:

  • Membership in good standing with the applicable regulatory body
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
