Job Summary

As a Care Coordinator with Home and Community Care Support Services, you will play a vital role in connecting patients with the care and services they need to live independently at home. This is a rewarding opportunity to make a meaningful difference in people's lives and work in a collaborative team environment that values patient-centered care.

Key Responsibilities
  • Assess patient care needs and determine eligibility for services in collaboration with patients and their families.
  • Link patients with service providers and coordinate care plan delivery.
  • Establish a helping relationship with patients and their families to ensure their values and preferences are respected.
  • Balance patient needs with available resources to provide accessible and responsive care.
Requirements
  • Membership with the applicable regulatory body (College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
  • 2+ years of recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Effective time management, prioritization, and organizational skills for a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports, and forms.
  • A valid driver's license and access to a reliable vehicle.
  • Proficiency in a Windows environment.
Preferred Qualifications
  • Experience working with diverse patient groups (multicultural, homeless, palliative, acquired brain injury, etc.).
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
