About the Job

We are seeking a compassionate and dedicated Continuing Care Assistant to join our team at Shannex's Transitional Care Centre in West Bedford, Nova Scotia. As a Continuing Care Assistant, you will play a vital role in providing personalized care and support to patients who require additional time and services to transition back to their previous living situation.

Key Responsibilities:

  • Provide personal care and assistance with activities of daily living, such as bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient's care plan
  • Collaborate with the patient and other members of the interdisciplinary team in the development of individualized patient care plans
  • Assist in the planning and implementation of patient leisure activities on a daily basis
  • Assist with light housekeeping duties
  • Perform delegated nursing acts according to company policy and scope of practice
  • Communicate and report relevant information regarding patient care or safety to Supervisor

About You

We are looking for a candidate who:

  • Has a valid Continuing Care Certification or Provincial Personal Care Worker course or equivalent
  • Is registered in Nova Scotia's CCA registry as per the Continuing Care Assistants Registry Act
  • Can work a schedule of flexible hours and shifts inclusive of days, nights, weekends, and holidays
  • Can provide a clear criminal record with vulnerable sector screening
  • Has previous working experience with elderly in long-term care or a senior living environment (an asset)
  • Has valid CPR & First Aid Certification (preferred)
