Summary: Experienced registered nurse sought to optimize transitions from hospital to home for frail adults and seniors with complex needs.

We are seeking a skilled Registered Nurse to join our team as a Rapid Response Nurse. In this role, you will play a critical part in ensuring that patients receive the best possible care after being discharged from the hospital.

The Rapid Response Nurse is responsible for providing in-home nursing visits within 24 hours of hospital discharge for complex/chronic patients. This includes reviewing the discharge care plan, confirming outstanding medical tests, and ensuring new prescriptions are filled and medication reconciliation is completed.

Key Responsibilities:

  • Provides in-home nursing visits within 24 hours of hospital discharge for complex/chronic patients
  • Reviews discharge care plan and confirms outstanding medical tests
  • Ensures new prescriptions are filled and medication reconciliation is completed
  • Triage patient priorities between new referrals and existing caseloads
  • Initiates contact with primary care physician and provides update on patient's acute care event and post-discharge regime
  • Recommends and facilitates follow-up visits with primary care physician as needed
  • Assessment, consultation, and treatment as required
  • Identifies patients requiring accelerated assessment and home care services and works with Care Coordinator to facilitate home assessment visits
  • Supports Care Coordinators in developing patient care plans and ensuring smooth transfer of care to on-going team
  • Acts as spokesperson as required, interpreting the role of Ontario Health atHome to patients, healthcare professionals, and the public
  • Works collaboratively with team members to provide timely triage of referred patients from in-patient units using standardized tools and processes
  • Participates in orientation of new staff and students
  • Assesses for and promotes a safe environment for caregivers, family members, and staff by adhering to health and safety policies/practices
  • Participates in establishing, maintaining, and monitoring standards for Care Coordinators, including committee work and quality initiatives

Requirements:

  • University degree in Registered Nursing
  • Registered Nurse in good standing with the College of Nurses of Ontario
  • Working knowledge of community resources and roles of healthcare professionals
  • Knowledge of Ontario Health atHome priorities, policies, practices, and service standards (an asset)
  • Minimum of 2 years of relevant experience (pediatrics or geriatrics) as a Registered Nurse
  • Proficiency with computerized information systems
  • Excellent knowledge of healthcare-related legislation and practices
  • Working knowledge of the nursing process, consultation process, program planning, and crisis management

Preferred Qualifications:

  • Emergency/critical care experience (an asset)
  • Community nursing experience (an asset)
  • Completion of Critical Care Course in area of specialty (an asset)
  • CNA certification in area of specialty (an asset)
  • Fluency in a second language (an asset)
