Mental Health Case Manager

Job Summary

We are seeking a highly skilled and compassionate individual to join our team as an Intake Facilitator for Mental Health Services. This role requires strong assessment and coordination skills, with the ability to work independently and as part of a multi-disciplinary team.

Key Responsibilities
  • Screen and coordinate initial service requests for all programs within Mental Health and Addictions Services
  • Provide screening, triage, and assessments for urgency/risk, and coordinate case assignments
  • Facilitate crisis response and short-term support for services to clients presenting with mental health and addictions issues
  • Consult with Community Agencies and Programs regarding services available in Mental Health and Addiction Services
Requirements
  • Bachelor of Social Work (BSW) or equivalent
  • Valid Class 5 driver's license
  • Experience working with mental health and addictions clients
  • Strong assessment and coordination skills
  • Ability to work independently and as part of a team
Preferred Qualifications
  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
  • HSAS membership
