Hamilton, Ontario, Canada DTOC III Long Term Care LP Full time

We are seeking a Personal Support Worker (PSW) to provide personal care and support to residents in a healthcare setting.

The ideal candidate is a warm and loving person who makes meaningful connections with those they are caring for. They will work closely with the Nursing Restorative Therapist to enhance restorative care and promote independence of activities of daily living.

The PSW will be responsible for:

  • Providing personal care and assistance with activities of daily living for residents
  • Directing and guiding residents in the provision of activities of daily living, ensuring to maximize their independence and allowing for choice and self-determination of their care
  • Monitoring resident's wellbeing and reporting changes in their physical and emotional conditions to registered staff
  • Accurately documenting resident information and care provided in electronic health records
  • Ensuring the health and safety of residents, families, and coworkers
  • Participating in resident care conferences and providing information to aid in the development of care plans
  • Using a Gentle Persuasive Approach (GPA) when dealing with and managing residents with expressive behaviors
  • Assisting with meal service and nourishment delivery to residents and providing feeding assistance as required

The PSW will also be responsible for:

  • Providing a clean, safe, and secure environment for residents during the provision of care
  • Ensuring personal care items are clean, labeled, and accessible to residents
  • Using safe transfer techniques and mechanical lifts in accordance with policies
  • Participating in quality improvement initiatives to enhance resident care and services

Qualifications:

  • PSW Certification from a program that is at least 600 hours in duration
  • Experience with person-centered care philosophy/models
  • Ability to communicate both orally and in writing
  • Good interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Must provide a complete and current Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
