**Forensic Community and Liaison Social Worker** A Forensic Community and Liaison Social Worker is required to provide comprehensive social work services to clients with complex mental health and substance use challenges in a forensic setting.

Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop an Integrated Treatment Plan. Provide community case coordination and advocacy services, collaborating with other health and social service providers to ensure comprehensive care. Maintain accurate and concise documentation of client information and provide written reports as required.

  • Provide clinical services, information, and support to clients, including teaching about mental illness, medication management, and treatment options.
  • Facilitate access to psychiatric and social services through referrals and site visits, and assist with community reintegration and prosocial development.
  • Conduct or participate in pre-release planning conferences and consult with the clinical treatment team to determine client needs and safety risks.
